摘 要:目的提出一种新的泪滴角(teardrop.gle,TDA)测量方法,并分析腕关节屈伸位置不同对测量值的影响。方法过泪滴的顶点以最短距离向月骨作线段与月骨相交于一点,过该点以月骨近端关节面的弧为基础做圆,将过该点的圆的切线作为泪滴的中轴线。其与桡骨纵轴线的夹角为TDA。本研究收集60个正常腕关节侧位片,根据摄片时腕关节背伸、中立和掌屈位将其分成三组,分别采用以上方法测量TDA,并分析腕关节屈伸对TDA测量有无影响。结果TDA值:背伸位为(53.39±5.88)°、中立位(53.54±6.49)°、掌屈位(52.86±5.44)°,三组侧位片TDA值之间的差异无统计学意义。结论新的方法简单、易操作,不受腕关节屈伸位摄片的影响,可进行临床推广。Objective To present a new method for measuring the tear drop angle (TDA) and analyze the influence of wrist flexion and extension on the measurements. Methods From the vertex of the tear drop a line was drawn to intersect with the lunate at the shortest distance. From the intersection point a template was used to trace a circle to match the arc of the proximal articular surface of the lunate. The tangent to the circle which passed through this point was defined as the axis of the tear drop. The TDA was formed between this axis and the central axis of the radial shaft. Sixty lateral X-rays of normal wrists were collected and divided into three groups: dorsiflexion, neutral and palmar flexion. TDAs were measured with the new method and the variances between these groups were analyzed. Results The values of TDA were (53.39 ± 5.88) ° in dorsiflexion, (53.54 ± 6.49)°in neutral and (52.86 ± 5.44)° in palmar flexion of the wrist. There were no statistically significant differences between the three groups. Conclusion The new TDA measuring method is simple and easy to manipulate, and not affected by the position of the wrist. Therefore, it can be useful in clinical practice.
