302例门急诊输液不合理处方干预结果分析

摘　　要：目的通过分析门急诊不合理处方干预结果,提高药师干预医师用药的成功率,促进合理用药。方法统计门急诊药房药师遇到的不合理用药干预情况,按不同干预人员、干预科室、干预问题进行分类分析。结果共干预302例,成功244例,成功率为80.79%。中、高级职称药师和初级职称药师干预成功率分别为90.63%和79.63%;急诊科干预次数最多,成功率(88.17%)相对较高;溶剂选择的干预最多,成功率为83.33%,超剂量用药、配伍禁忌及其他干预成功率均较低。结论重视门急诊处方干预,可促进门急诊医师与药师的配合;加强门急诊临床药师的培养,可提高药师自身专业水平和干预能力。Objective To increase the success rates of pharmacists＇ intervention on doctors＇ medications and to promote the reasonable drug use by analyzing the intervention results of the unreasonable prescriptions in the outpatient and emergency departments. Methods The intervention situation on the irrational medications in the outpatient and emergency pharmacy encountered by pharmacists were sta- tistically analyzed from the three aspects of different intervention personnels, departments and issues. Results Three hundred and two cases were performed the intervention, in which 244 cases succeeded with the success rate of 80.79%. The success rates of interven- tions by senior and junior pharmacists were 90.63% and 79. 63%, respectively; the intervention times of the emergency department was maximal with the relatively higher success rate （88.17%）; the interventions on solvent selection were maximal with highest success rates （83.33%）, other intervention issues, such as over-dose medications, incompatibility and other conditions, had the lower success rates. Conclusion Several suggestions are provided, which including emphasizing the prescription intervention in the outpatient and e- mergency departments, promoting the coordination between doctors and pharmacists in the outpatient and emergency departments, reinforc- ing the cultivation of clinical pharmacists in the outpatient and emergency departments and improving the academic levels and interven- tion skills of pharmacists.
