摘 要:目的:探讨多囊卵巢综合征(Polycystic ovary syndrome,PCOS)大鼠卵巢局部和单卵泡中抗苗勒管素(Anti-Mullerian hormone,AMH)表达和胰岛素抵抗(Insulin resistance,IR)之间的相关性。方法:孕激素联合促绒毛膜性腺激素(Human chorionic gonadotropin,HCG)法制作PCOS大鼠模型,根据胰岛素抵抗稳态模型(Homeostasis model assessment of insulin resistance,HOMA-IR)值筛选实验对象。将其分为:PCOS无IR组,PCOS合并IR组和对照组。采用免疫组织化学法对不同组大鼠的卵巢组织局部和单卵泡进行胰岛素受体(Insulin receptor,Ir)和AMH表达强度计算阳性单位行半定量分析。结果:对照组、PCOS无IR和PCOS合并IR组三组相比,血清空腹胰岛素(Fasting insulin,Fins)均有显著性差异(P<0.05);三组间卵巢局部AMH表达呈递增型阶梯样分布(P<0.05);而Ir表达无统计学差异(P>0.05);卵巢局部AMH和Ir表达的相关性分析显示二者无明显相关性(r=0.000,P>0.05);在单卵泡的表达中,三组的AMH及Ir表达强度均无统计学差异(P>0.05);相关性分析显示AMH与Ir表达强度无明显相关性(r=0.011,P>0.05)。结论:PCOS卵巢局部AMH的高表达与高胰岛素血症的发生具有相关性,二者的相互作用可能与PCOS的高雄激素特征有关。Objective:To study the correlation between the expression of Anti-Mullerian hormone (AMH) and insulin resist- ance (IR) degree in local ovary and single follicle induced by polyeystic ovary syndrome (PCOS) model. Methods: PCOS rat model was constructed by progestogen and HCG combination method. Serum AMH, fasting plasma glucose (FPG) , fasting insulin (FINS) levels and HOMA index were measured. According to HOMA index, the rats were divided into PCOS with and without IR group. Mean- while, normal control group was set up for analysis. The expression of AMH and insulin receptor was measured in local ovarian and sin- gle follicle with immunohistochemical method for semi-quantitative analysis. Results: The serum FINS level was significantly higher in PCOS with IR group compared with that in control group and PCOS without IR group ( P 〈 0.05 ) ;in ovarian local AMH expression a- mong the three groups were incremental step distribution( P 〈 0.05 ) but the expression of insulin receptor was not significantly different ( P 〉 0.05 ). There was no significant correlation between local ovarian AMH level and insulin receptor expression ( r = 0. 000, P 〉 O. 05 ). AMH and insulin receptor in single follicle had no significant difference ( P 〉 0.05 ) and correlation ( r = O. 011, P 〉 0.05 ). Conclusion: In PCOS rat model, ovarian local AMH expression are regulated by insulin level in serum. The interaction may be associ- ated with high androgen feature of PCOS.
