小针刀配合手法松解治疗膝骨关节炎的临床研究

摘　　要：目的:观察小针刀配合手法松解治疗早中期膝骨关节炎的近远期疗效,初步探讨治疗的作用机制。方法:选取膝骨关节炎Kallgren-Lawrence分度Ⅱ度和Ⅲ度各100例,将200病例随机分成两组,治疗组和对照组各100例,分别对患者在治疗前、治疗4周、治疗24周进行Lequesne和疼痛视觉模拟评分(VAS)评定,观察治疗的近远期疗效。结果:两组治疗后的Lequesne评分和VAS评分均较治疗前降低,其中治疗4周的评分两组比较无差异,24周的两种评分针刀组均优于对照组(P<0.01)。针刀组中,Ⅱ度病例的4周和24周的改善率均比Ⅲ度病例要高(P<0.01)。结论:针刀组取得与对照组同样的近期疗效,而远期疗效较对照组更好。针刀组的治疗结果与病情严重程度有关,对Ⅱ度病例的治疗效果优于Ⅲ度病例。Objective： To observe the short and long-term curative effects of acupotomy combined with release manipulation on the early and intermediate-stage knee osteoarthfitis, and preliminarily investigate the action mechanism. Methods： 200 of knee osteoarthritis, 100 cases of lI degree and 100 cases of llldegree graded by Kallgren-Lawrence were selected and randomly divided into treatment group and control group, with 100 cases in each group. The patients in the treatment group were treated with acupotomy combined with release manipulation, and those in the control group by oral diclofenac sodium. The Lequesne indexes of knee function and VAS has been evaluated for the patients before treatment, 4 and 24 weeks after the treatment in order to observe the short and long-term curative effects of the two therapies on the early and intermediate-stage knee osteoarthritis. Results： The Lequesne and VAS scores for the two kinds of therapies were lower than those of before treatment and there was no difference between the scores for the two therapies of 4 weeks. But the scores of 24 weeks for treatment group were lower than those for control group （P〈0.01）. In the treatment group, II degree cases get more improvement rate than the III degree cases both at 4 and 24 weeks after the treatment （P〈0.01）. Conclusion： The treatment group has the same good effects with control group of short-term therapy on early and mediate-stage knee osteoarthritis. The treatment group is better than the control group for long-term treatment and the treatment results depend on the severity of the disease. The treatment effects on II degree cases are better than those on II degree cases.
