病毒性脑炎患儿发生心律失常危险因素分析

摘　　要：目的:探讨病毒性脑炎并发心律失常的影响因素。方法:选取本院内科2010年1月-2012年12月共收治的90例临床资料完整的病毒性脑炎患儿作为研究对象,对患儿临床特征与心律失常的关系进行单因素和多因素回归分析,对各危险因素与心律失常发生情况之间的相关性使用spearman相关性检验进行分析。结果:90例中有42例病毒性脑炎患儿出现心律失常65例次,发生率46.7%(42/90),包括各类早搏、阵发性室上性心动过速、心房颤动、窦性心动过缓或过速、一度或二度房室传导阻滞(Ⅰ度或Ⅱ度AVB)等。单因素分析显示:频繁抽搐(抽搐反复发作时间超过3 d)、意识障碍、低年龄、心肌肌钙蛋白(cTnI)升高、低血钾与病毒性脑炎并发心律失常相关联(P<0.01);多因素Logistic回归分析显示:频繁抽搐、心肌肌钙蛋白Ⅰ升高与病毒性脑炎并发心律失常显著独立相关(P<0.05)。相关性分析结果显示:频繁抽搐与心律失常发生呈正相关(r=0.250,P=0.027),心肌肌钙蛋白I与心律失常发生呈正相关(r=0.382,P=0.001)。结论:频繁抽搐、心肌肌钙蛋白Ⅰ升高是病毒性脑炎发生心律失常的危险因素。Objective：To explore the risk factors of viral encephalitis complicated with arrhythmia.Method：90 children with viral encephalitis in our hospital from January 2010 to December 2012 were selected as research objects. All clinical data were reviewed,linear relationship analysis with spearman correlation coefficients was used to study the correlations of some factors.Result：42 arrhythmias occurred in 90 patients（46.7%）,arrhythmias consisted of premature complexes beat,supraventricular tachycardia,sinus tachycardia,sinus bradycardia,ⅠandⅡAVB et al.Single factor analysis showed that convulsions,consciousness obstruct,lower age,increase of cTnI,hypokalemia were risk factors of arrhythmias in children with viral encephalitis（P＆lt;0.01）.Multivariate logistic regression analysis showed that convulsions and increase of cTnI were important risk factors to viral encephalitis with arrhythmias（P＆lt;0.05）.Firstly in linear relationship analysis,arrhythmias was related to time of convulsions,and the correlation coefficient was 0.250（P=0.027）,secondly increase of cTnI was also related to arrhythmias,and the correlation coefficient was 0.382（P=0.001）.Conclusion：Convulsions and increase of cTnI are important risk factors to viral encephalitis with arrhythmias.
