糖肾灵治疗早期糖尿病肾病患者60例临床疗效观察

摘　　要：目的探讨以益气养阴、活血化瘀为治法组成的中药糖肾灵对早期糖尿病肾病(diabetic nephropathy,DN)的治疗效果。方法纳入我中心早期糖尿病肾病患者60例,随机分为对照组30例和治疗组30例。对照组采用常规西药治疗,治疗组在此基础上加用糖肾灵。连续治疗3个月,观察两组患者在症候疗效、血糖、糖化血红蛋白(glycosylated hemoglobin,HbA1)、尿白蛋白/肌酐比(urinary albumin to creatinine ratio,UACR)、肾小球滤过率(glomerular filtration rate,GFR)、血肌酐(serum creatinine,SCr)、血尿素氮(blood urea nitrogen,BUN)水平变化。结果两组在治疗前各项指标无显著差异;经3个月治疗后,治疗组各种证候疗效改善率明显高于对照组,并且显著降低患者血糖、HbA1、UACR及SCr水平,提高GFR。同时与治疗前相比较,治疗组UACR、SCr水平及GFR亦有明显改善。结论糖肾灵既能改善早期糖尿病肾病患者的血糖水平,又能延缓肾功能恶化,说明糖肾灵辅助治疗早期糖尿病肾病,其疗效优于单用西药治疗。OBJECTIVE To explore the therapeutic effects of Chinese medicine Tangshenling, which can increase function of spleen and kidney and remove blood stasis, on early diabetic nephropathy（DN）. METHODS Sixty patients with early DN who were admitted to the center were randomly divided into the control group treated and the treatment group, each group 30 cases. The conventional western therapy was given to all patients in both groups and Tangshenling was added in the treatment group for 2 months. The clinical symptomsbefore and after treatment and levels of blood glucose, glycosylated hemoglobin（HbA1）, urinary albumin to creatinine ratio （UACR）, glomerular filtration rate（GFR）, serum creatinine（SCr） and blood urea nitrogen（BUN） were observed. RESULTS Before treatment, there was no significant difference in all these indexes between the control group and the treatment group. After 2 months of treatment, the effective rates in clinical symptomsimprevement in the treatment group were higher than the control group, and blood glucose, HbA1, UACR and SCr were significantly decreased and GFR was obviously increased than the control group. Moreover, the blood glucose, HbA1, UACR, GFR and SCr were also significantly improved. CONCLUSION Tangshenling could evidently not only ameliorated the levels of blood glucose in patients with early DN but also postponed renal function deterioration. It suggests that treating with Chinese medicine Tangshenling combined withconventional western therapy has better effects than only western medicine.
