乌司他丁治疗急性左心衰竭患者临床疗效及对细胞免疫功能的影响

摘　　要：目的:研究乌司他丁治疗急性左心衰竭患者临床疗效并观察其对于患者细胞免疫功能的影响。方法:选取48例急性左心衰竭患者,随机分为研究组和对照组,其中研究组25例,对照组23例,两组均接受强心、利尿、扩血管以及抗感染、维持呼吸功能等常规治疗,研究组在常规治疗基础上加用乌司他丁治疗,观察两组疗效及治疗前后的细胞免疫功能。结果:治疗后两组的左室射血分数(LVEF)和心输出量(CO)均出现明显改善,而研究组治疗后的LVEF和CO均明显高于对照组。研究组治疗后CD4+、CD8+及CD4+/CD8+均较治疗前升高,NK细胞水平降低,对照组治疗后CD4+、CD8+水平升高,NK下降,CD4+/CD8+较治疗前无明显变化;研究组治疗后24、48h的CD4+和CD4+/CD8+均明显高于对照组,治疗后24、48h的NK细胞水平低于对照组;两组治疗前后均未出现明显不良反应,安全可靠。结论:急性左心衰竭患者多合并一定程度的免疫功能下降,乌司他丁可以有效改善患者的细胞免疫功能,进而提高疗效。Objective： To investigate clinical efficacy of Ulinastatin on patients with left ventricular failure and its impact on acute cellular immune function. Methods： In this study, 48 cases of acute left heart failure patients were divided randomly into study group and the control group with 25 cases in the study group and 23 patients in the control group. Both groups received cardiac, diuretic, vasodilator and anti-infection, maintaining respiratory function and other conventional treatment. Patients in the study group were also given ulinastatin based on conventional therapy. Efficacy and cellular immune function of the two groups were observed. Results： After treatment, LVEF and CO were improved in both groups but they were significant higher than those in the study group. After treatment, CD4^＋ and CD8^＋ were increased in the study group, but NK was decreased; and no significant changes in CD4^＋/CD8^＋ was observed. Comparing with the control group, CD4^＋, CD8^＋ and CD4^＋/CD8^＋ were all higher in the study group 24 and 28 hours after the treatment, but 24 h and 28 h NK were lower. No obvious negative reactions were observed in the two groups. Conclusion： Decreased immune function is common in patients with acute left ventricular failure, Ulinastatin can improve immune cell function in these patients, thus improving efficacy.
