摘  要:目的:探讨N-乙酰-β-D氨基葡萄糖甘酶(NAG酶)、γ-谷氨酰转移酶(γ-GT)和尿微量白蛋白(mAlb)联合检测对早期诊断肾损伤的临床应用价值。方法:采用终点法和速率法对113例尿蛋白定性阴性的患者和30例健康人群尿中NAG酶、γ-GT、mAlb进行检测,探讨三者联合检测对肾损伤早期诊断的价值。结果:DM组、HBP组、SLE组患者mAlb、NAG酶、γ-GT检测结果明显高于对照组,差异具有统计学意义(P<0.05);三种指标单项检测阳性率较低,两两联合检测会提高检测阳性率,但是与单项检测相比差异无统计学意义(P>0.05);三项联合检测明显提高检测阳性率,与单项及两项检测相比,差异具有统计学意义(P<0.05);相关性分析显示,mAlb、NAG酶、γ-GT之间呈正相关关系。结论:mAlb、NAG酶、γ-GT联合检测可以作为早期诊断肾损伤的指标,值得临床推广应用。Objective: To explore the clinical value of combined detection of urine trace protein enzymes,NAG enzyme, gamma glutamyl transfer enzymes in early diagnosis of renal injury. Methods: The end-point method and rate method were applied to detect urinary NAG enzyme, γ-GT, mAlb in 113 patients with negative qualitative urine protein results and 30 healthy volunteers. Results. urinary NAG enzyme, γ-GT, mAlb levels were significant higher in DM group, HBP group and SLE group than that in the control group (P〈0.05). Positive rate of detection of single parameter was lower than that of combined of two parameters, but the difference was not significant (P〈0.05). Positive rate of detection of three parameters together is significant higher than detection of single parameter (P〈0.05). Correlation analysis showed a positive correlation between mAlb, NAG enzyme, γ-GT. Conclusion. Combined detection of mAlb, NAG enzyme, γ-GT can reveal the severity of renal injury, and it's worthy of application in large scale.
