瑞舒伐他汀对急性冠脉综合征患者血清炎性因子、血脂水平及颈动脉斑块的影响

摘　　要：目的探讨瑞舒伐他汀对急性冠脉综合征(ACS)患者血清炎性因子、血脂水平及颈动脉斑块的影响。方法选择98例ACS患者,随机分为两组,对照组49例,给予常规治疗;研究组49例,在常规治疗的基础上加用瑞舒伐他汀10 mg/d,睡前口服,连用6个月,分别在治疗前后检测两组患者血清炎性因子和血脂水平,并观察颈动脉斑块的变化情况。结果治疗后,两组患者血清TNF-α、IL-6、MMP-9水平较治疗前均明显下降,但研究组下降更明显,差异有统计学意义(P<0.05)。研究组治疗后颈动脉斑块大小、数量、厚度和IMT均较治疗前显著改善(P<0.05),而对照组治疗后虽然斑块大小、数量、厚度和IMT均较治疗前改善,但差异无统计学意义(P>0.05)。结论 ACS患者应用瑞舒伐他汀不仅有调血脂作用,还有抗炎作用,能够改善斑块炎症反应,稳定粥样斑块。Objective To investigate the influences of rosuvastatin on serum inflammatory factors ,blood lipid level and carotid plaque in patients with acute coronary syndrome （ ACS） .Methods Ninety-eight cases of ACS were randomly divided into two groups ,49 cases in the control group were treated with conventional ACS therapy ,while 49 cases in the research group took rosuvastatin （10 mg per day before bedtime ,6 months） besides conventional therapy . The serum inflammatory factors and blood lipid level were detected before and after treatment in both groups ,and the changes of carotid plaque were observed .Results The levels of tumor necrosis factor α（TNF-α）,interleukin 6（IL-6） and matrix metalloproteinase 9 （ MMP-9 ） after treatment were significantly lower than those before treatment in both groups,but the decrease in the research group was more significant （P〈0.05）.The carotid plaques size,number, thickness and intima-media thickness （ IMT） after treatment were significantly lower than those before treatment in the research group （P〈0.05）.In the control group,the carotid plaques size,number,thickness and intima-media thickness （IMT） after treatment improved compared with those before treatment,which showed no statistical significance（P〉0.05）. Conclusion Rosuvastatin can improve blood lipid and plaques inflammation in ASC patients ,with anti-inflammatory effect.It can make atheromatous plaque stable .
