摘 要:目的探讨抗菌药物的管理策略,指导抗菌药物合理应用。方法抽查我院实施抗菌药物应用强化管理的出院病例1568份作为观察组,抽查我院实施抗菌药物应用强化管理前的出院病例1482例作为对照组。对比分析两组患者抗菌药物应用情况及医院感染情况。结果观察组抗菌药物使用率31.1%,显著低于对照组(57.8%)(χ2=219.329,P=0.000);观察组有指征用药(94.0%)显著高于对照组(76.3%)(χ2=71.428,P=0.000);观察组不合理用药占6.0%,显著低于对照组(23.7%)(χ2=67.402,P=0.000);观察组联合用药13.9%,对照组14.1%,两组相比无显著差异。对照组医院感染发生率为5.6%,显著高于观察组(2.6%)(χ2=17.414,P=0.000)。结论强化抗菌药物应用管理制度,不仅能提高并规范临床抗菌药物的合理使用,同时还能减少医院感染的发生。Objective To explore the management strategy of antibiotics administration and to provide a guidance for proper application of antibiotics. Methods 1568 discharged cases after the performance of enforced management of antibiotics were selected to be the observation group,while 1482 cases before the performance were selected to be the control group;a contrast study was made to the administration of antibiotics and the occurrence of nosocomial infection in the 2 groups. Results The rate of antibiotics administration in the observation group was 31.1% and was much lower than that in the control group(57.8% ) (X^2 =219. 329,P = 0.000). The rate of antibiotics administration with indications in the observation group was 94.0% and was much higher than that in the control group (76.3%)(X^2 = 71. 428 ,P = 0.000). The rate of irrational administration of antibiotics in the observation group was 6.0% and was much lower than that in the control group (23.7%) (X^2 = 67. 402, P = 0. 000). The rate of co-administration was 13.9% in the observation group and 14.1% in the control group, the difference between the 2 groups was of no significance. The occurrence of nosocomial infection in the control group was 5.6% and was obviously higher than that in the observation group(2.6% ) (X^2 = 17. 414, P = 0. 000). Conclusions The enforced management can improve and standardize the rational administration of antibiotics in clinic and can decrease the occurrence of nosocomial infection.
