摘 要:目的 探讨后路椎弓根螺钉内固定治疗多节段胸腰椎骨折(MSF)的疗效.方法 回顾性分析2008年1月~2011年12月64例行后路椎弓根螺钉内固定术多节段胸腰椎骨折患者临床资料.比较术前及术后患者的椎体高度和后凸角的改变情况,Frankel脊髓评分判定神经功能,评估临床疗效.结果 所有患者均获得随访,术前伤椎椎体前缘高度为(43.7±5.1)%,后凸角为(31.2±4.3)°;术后末次随访时伤椎椎体前缘高度恢复到(91.7±4.7)%,后凸角为(7.9±3.1)°,手术前后比较,差异有统计学意义(P<0.01).根据Frankel分级,多数患者的神经功能有所恢复.结论 在对多节段胸腰椎骨折患者进行临床治疗时,后路椎弓根螺钉内固定术有利于恢复多节段伤椎高度、胸腰椎的生理弯曲和神经功能,具有良好的临床疗效.Objective To investigate the effects of posterior pedicle screws fixation in treatment of multiple-llevels thoracolumbar fracture. Methods A retrospective analysis was made on the date of 64 patients with the multiple-levels thoracolumbar fracture treated by posterior pedicle screws fixation. The difference of vertebral body height, angle of kyphosis deformity before and after operation were compared, and the Frankel score were adopted to evaluate surgery eftlcaey. Results All cases were foIIowed up. The vertebral body height was ( 43.7 ± 5.1 ) % and the angle of kyphosis was ( 31.2 ± 4.3 ) ° before the operation. After operation, the vertebral body height was (91.7 ± 4.7 ) %, and the angle of kyphosis was (7.9 ± 3.1 ) °. The anterior vertebral height and kypbosis angle were significantly improved after the operation (P 〈 0. 01 ). According to the Frankel score, the neural function of majority of patients was apparent recovery. Conclusions Posterior pedicle screws fixation is an effective method of treatment of multi-levels thoracolumbar fractures. This method can recovery injured vertebrae height, physiological postural contour of the spine, and is benefit to neuronal recovey.
