镇江市2011年医疗卫生机构控烟干预效果评价

摘　　要：目的对镇江市医疗卫生系统"无烟单位"创建过程中实施控烟措施进行效果评价。方法通过随机抽样确定在7家二级以上医院开展调查,在控烟干预前后,通过无烟环境观察、医务人员调查以及就诊者拦截调查的方法,对干预效果进行评价。结果干预后,有控烟宣传资料发放的单位比例从85.7%上升到了100.0%,重点区域内都张贴禁烟标识的比例达到100.0%,室外吸烟区设置率达从85.7%上升到了100.0%。就诊者在医院见到禁烟标志、控烟宣传、有人主动宣传禁烟知识以及看到有人劝阻吸烟的比例均有所上升,闻到烟味的比例有所下降。干预后,医务人员吸烟率下降、戒烟率上升,吸烟者中有戒烟意愿的比例有所提高。认为单位禁烟规定执行情况良好的比例由干预前的56.8%上升为干预后的71.0%,支持室内全部禁烟的比例由干预前的84.5%上升到干预后的94.5%。结论医院内控烟宣传氛围浓厚,"无烟医院"创建效果明显。然而,室内仍存在吸烟现象,应坚持长效管理,巩固无烟单位创建成果。Objective This study was to assess the effect of smoking control during the tobacco-free campaign in hospitals of Zhenjiang City, 2011. Methods Random sampling method was used to select 7 hospitals of second class or above for smoking control in- tervention. Before and after the intervention, the environment of the hospital settings was inspected, and the medical personnel and patients were surveyed to assess the effect of intervention. Results After the intervention, the percentage of hospital units that deliver smoke-free pamphlets and set designated outdoor smoking areas increased from 85.7% to 100.0%; no-smoking signs were posted in all critical areas in the hospitals. More patients paid attention to no-smoking signs and tobacco control pamphlets, and took initiative to encourage smokers to quit smoking. In contrast, less patients smelled smoke. After the intervention, less medical staff smoked and more quitted or planned to quit smoking. The percentage of hospitals complying with tobacco-free regulations increased from 56.8% to 71.0% after the intervention. Percentage of medical staff who support tobacco-free environment increased from 84.5% to 94.5. Conclusion The effect of tobacco-free campaign is evident. However, smoking indoors still exists. Long-term effort should be paid to consolidate the achievement of tobacco-free environment.
