摘  要:目的:探讨核转录因子(NF-κB)、胎盘生长因子(PLGF)、肿瘤坏死因子(TNF-α)与子痫前期发病的关系,为临床诊断及治疗提供依据。方法:选取60例子痫前期患者(观察组,其中轻度30例,重度30例)和30例正常孕妇(对照组),采用酶联免疫吸附法(ELISA)对观察组和对照组患者的血清TNF-α和PLGF水平以及胎盘中NF-κB、TNF-α和PLGF水平进行检测,对三者的相关性进行分析。结果:观察组患者胎盘组织中TNF-α和NF-κB表达水平显著高于对照组(P<0.05),观察组患者血清PLGF表达水平显著低于对照组(P<0.05)。观察组中重度子痫前期组患者TNF-α和NF-κB表达水平显著低于轻度子痫前期组(P<0.05),二者PLGF表达水平相比无统计学差异(P<0.05)。且NF-κB及TNF-α与PLGF表达强度呈显著负相关,NF-κB与TNF-α表达强度呈显著正相关。观察组患者血清TNF-α水平显著高于对照组(P<0.05),观察组患者血清PLGF的水平显著低于对照组(P<0.05)。观察组中重度子痫前期组患者血清TNF-α水平较轻度子痫前期组显著增高(P<0.05),血清PLGF水平显著低于轻度子痫前期组(P<0.05),且观察组患者血清TNF-α与PLGF水平呈显著负相关。结论:在子痫前期发生过程中,TNF-α、PLGF、NF-κB三种细胞因子间存在一定的联系,可能通过相关机制参与子痫前期发病,并且三类物质与子痫前期的进展及其病情严重程度有密切关系。Objeαive: To explore the relationship between nuclear transcription faαor (NF - KB), placental growth faαor (PL- GF), tumor necrosis faαor - α (TNF - α) and onset of preeclampsia, provide a basis for clinical diagnosis and treatment. Methods: Sixty patients with preeclampsia ( observation group, including 30 patients with mild preeclampsia and 30 patients with severe preeclampsia) and 30 normal pregnant women (control group) were seleαed; ELISA was used to deteα the serum levels of TNF -α and PLGF, placental levels of NF - KB, TNF - α and PLGF in the two groups ; their correlation was analyzed. Results : The expression levels of TNF - α and NF - KB in placental tissue of observation group were statistically significantly higher than those in control group ( P 〈 0. 05 ) , the expression level of PLGF in serum of observation group was statistically significantly lower than that in control group ( P 〈 O. 05 ) . The expression levels ofTNF - α and NF - KB in severe preeclampsia subgroup were statistically significantly lower than those in mild preeclampsia subgroup ( P 〈 0. 05), but there was no statistically significant difference in the expression level of PLGF between severe preeclampsia subgroup and mild preeclampsia subgroup ( P 〈 O. 05 ) . There was a significantly negative correlation between NF - KB, TNF - α expression levels and PLGF expression level, there was a significantly positive correlation between NF - KB expression level and TNF - α expression level. The expres- sion level of TNF - α in serum of observation group was statistically significantly higher than that in control group ( P 〈 O. 05 ), the expression level of PLGF in serum of observation group was statistically significantly lower than that in control group ( P 〈 O. 05 ) .
