摘  要:目的:探讨脂联素受体2(AdipoR2)的表达变化在子痫前期发病机制中的作用。方法:收集20例正常妊娠妇女和20例子痫前期患者大网膜脂肪组织及胎盘组织,分别运用实时定量PCR技术和Western blot印迹法检测AdipoR2 mRNA及其蛋白质的表达;同时测定空腹血糖、空腹胰岛素、体重指数等临床指标。结果:①子痫前期患者存在胰岛素抵抗和高胰岛素血症。②与正常妊娠妇女比较,子痫前期患者胎盘组织中AdipoR2 mRNA及其蛋白质的表达水平均显著升高,大网膜脂肪组织中均显著降低,差异均有统计学意义(P<0.05)。③两组孕妇胎盘组织中及正常妊娠妇女大网膜脂肪组织中AdipoR2 mRNA的表达水平与BMI、FIN、FPG、HOMA-IR均无相关性(P>0.05),但子痫前期患者大网膜脂肪组织中AdipoR2 mRNA的表达水平与BMI、FIN、HOMA-IR均呈显著负相关(P<0.05)、与FPG无相关性(P>0.05)。结论:AdipoR2在胎盘组织中的表达升高,在大网膜脂肪组织中的表达降低,可能在子痫前期的发病机制中发挥重要作用。子痫前期患者大网膜脂肪组织中AdipoR2的表达下降可能是受到超重、低ADP血症、高胰岛素血症及胰岛素抵抗的影响。Objective : To explore the role of adiponectin receptor 2 (AdipoR2) in pathogenesis of preeclampsia. Methods : The omental adipose tissue and placental tissue of 20 normal pregnant women and 20 patients with preeclampsia were collected. AdipoR2 mRNA level was detected by quantitative RT - PCR and AdipoR2 protein level was detected by Western blot. Fasting blood glucose, fasting insulin and body mass index (BMI) were detected simultaneously. Results: Insulin resistance and hyperinsulinism existed in the preeclampsia pa- tients. Compared with normal pregnant women, the expression levels of AdipoR2 mRNA and its protein in placental tissue of patients with preeclampsia increased significantly, but their expression levels in omental adipose tissue decreased significantly, there were statistically sig- nificant differences ( P 〈 0. 05 ) . There were no correlation between AdipoR2 mRNA expression levels in placental tissue of the two groups, omental adipose tissue of normal pregnant women and BMI, fasting insulin, fasting blood glucose, HOMA - IR (P 〉 0. 05 ), but there was a significantly negative correlation between AdipoR2 mRNA expression level in omental adipose tissue of patients with preeclampsia and BMI, fasting insulin, HOMA - IR ( P 〈 0. 05 ), which was not correlated with fasting blood glucose (P 〉 0. 05 ) . Conclusion : Overexpression of AdipoR2 in placenta tissue and downregulation of AdipoR2 in omental adipose tissue may play important roles in pathogenesis of preeclamp- sia. The downregulation of AdipoR2 in omental adipose tissue may be affected by overweight, hypoadiponectinemia, hyperinsulinism and in- sulin resistance.
