急性呼吸窘迫综合征患儿血清TNF-α和IL-6水平检测的临床意义

摘　　要：目的观察急性呼吸窘迫综合征(ARDS)患儿血清肿瘤坏死因子α(TNF-α)、白介素6(IL-6)的变化,探讨其临床意义。方法以90例ARDS患儿和45例健康对照组为研究对象,根据2012年ARDS柏林标准以病情严重程度将患儿分为轻、中、重三组,采用酶联免疫吸附实验方法(ELISA)测定各组血清TNF-α和IL-6水平。结果 ARDS组患儿血清TNF-α和IL-6水平均高于健康对照组;重度ARDS组及中度ARDS组患儿血清TNF-α和IL-6水平均高于轻度ARDS组(P<0.05),重度ARDS组患儿血清TNF-α和IL-6水平均高于中度ARDS组(P<0.05)。结论 TNF-α、IL-6在儿童急性呼吸窘迫综合征发病中起重要作用,且与ARDS的发生、发展和严重程度密切相关。【Objective】To investigate the relationship of serum tumor necrosis factor-α（TNF-α） and interleukin-6（IL-6） in children with acute respiration distress syndrome（ARDS）.【Methods】Serum TNF-α and IL-6 contents were measured by ELISA in 90 children with ARDS and 45 healthy children as control. 【Results】Serum levels of TNF-α and IL-6 in children with ARDS were significantly higher than those in healthy children（all P 0.05）. Compared with mild ARDS, Serum levels of TNF-α and IL-6 were significantly higher in children with moderate and severe ARDS（all P 0.05）. Serum levels of TNF-α and IL-6 in children with severe ARDS were significantly higher than those in children with moderate ARDS（all P 0.05）. Serum levels of TNF-α showed a positive correlation with serum levels of IL-6 in children with ARDS.【Conclusions】Serum levels of TNF-α and IL-6 may be related with the pathogenesis and development of ARDS.
