螺旋CT与低场MRI在成人股骨头坏死早期诊断中的应用价值  被引量:10

摘  要:目的探讨CT和MRI在成人股骨头坏死(avascular necrosis of fem-oral head,ANFH)早期诊断中的应用价值。方法以螺旋CT与低场MRI对47例(84髋)ANFH可疑患者作诊断检查。结果 47例受检对象中,股骨头正常4例(7髋),早期ANFH 33例(58髋),中、晚期ANFH 10例(19髋);CT与MRI阳性率分别为83.1%和97.4%,其中早期病变诊断准确率,CT 69.0%,MRI 96.5%,差异有统计学意义(P<0.05)。结论螺旋CT和与低场MRI都是诊断ANFH的可靠手段,相对于CT,MRI分期更准确,对早期ANFH诊断较敏感,当为首选检查方法。Objective To evaluate the clinical value of CT and MRI in the early diagmosis of the adult femoral head necrosis (ANFH) . Method 47 patients (84 hips) ANFH suspected patients were examined by Spiral CT and low-ifeld MRIrespectively.Results 4 cases(7 hips) with normal femoral head, cases (58 hips) with early ANFH33, 10 cases (19 hips) with middle and late ANFH; CT and MRI-positive rate was 83.1% and 97.4% respectively. The early lesion diagnostic accuracy of CT and MRI was 69.0%and 96.5%, the difference was statistically signiifcant (P〈0.05). Conclusion Spiral CT and low-ifeld MRI are the reliable method for the diagnostic of ANFH. Compred with CT, MRI staging is more accurate, more sensitive for early diagnosis of ANFH. MRI should be the preferred method of ANFH examination.
