手术治疗Lisfranc关节损伤的临床体会

摘　　要：目的探讨手术治疗Lisfranc关节损伤的临床疗效。方法 2008年2月~2012年1月,手术治疗Lisfranc关节损伤30例,按Myerson分型:Ⅰ型8例,Ⅱ型14例,Ⅲ型8例。伤后至手术时间2 h^35 d,平均7 d。结果切口均Ⅰ期愈合,30例患者均得到术后随访,随访时间6~47个月,平均22个月。按照AOFAS中足评分标准,90~100分11例,80~89分15例,70~79分4例,平均88分,优良率86.7%。结论手术治疗Lisfranc关节损伤能够重建跖跗关节复合体的稳定性,可取得满意的复位和临床疗效。足内侧柱以位置螺钉坚强固定,足外侧柱以克氏针弹性固定。[Objective] To study the clinical results of surgical treatment for Lisfranc fracture-dislocation. [ Methods ] From February 2008 to January 2012, 30 patients with Lisfranc injm＇ies were treated by surgieal opera- tion. According to Myerson＇s classification, there were 8 lype- I fractures, 14 type-Ⅱ, 8 lype-Ⅲ. All the patients accepted the operation in average 7 days（2 hours to 35 days）. [Results] All the wounds were healed primarily. All patients were followed up for 6 to 47 months （averaged 22 months）. According to the American Orthopaedic Foot and Ankle Soeiety（AOFAS） midfoot score system, l 1 cases scored 90 to 100, 15 cases scored 80 to 89, 4 eases scored 70 to 79. Good to excellent rate： 86.7%. [Conclusion] Lisfranc injuries can be surgically treated well. The medial- middle column of the foot should be fixed firmly and the fixation of the lateral column should be elastic.
