孟鲁司特联合沙美特罗替卡松治疗儿童中度哮喘的疗效及对IL-13和TNF-α水平的影响

摘　　要：目的探讨孟鲁司特联合沙美特罗替卡松治疗儿童中度哮喘的临床疗效及对白细胞介素-13(IL-13)和肿瘤坏死因子-α(TNF-α)水平的影响。方法 66例中度哮喘患儿随机分成A组(单纯给予沙美特罗替卡松)和B组(联合应用沙美特罗替卡松联合孟鲁司特),另选取健康儿童33例作为C组。观察A、B组临床疗效及对IL-13和TNF-α水平的影响。结果 A组和B组总有效率分别为72.7%和93.9%,两组治疗12周后第1秒用力呼气容量(FEV1)/用力肺活量(FVC)及最大呼气峰流速(PEF)%均明显提高,B组提高更明显(P<0.05);治疗前A组和B组患儿IL-13和TNF-α水平均明显高于C组(P<0.05),治疗12周后A、B组患者IL-13和TNF-α水平均有明显下降,B组下降更为明显(P<0.05)。结论孟鲁司特联合沙美特罗替卡松能明显改善中度哮喘患儿肺功能及炎症状态,疗效明显优于单用沙美特罗替卡松,值得临床推广。Objective To explore the efficacy of montelukast combined with salmeterol fluticasone and influence on serum levels of inter- leukin - 13 （ IL - 13 ） and tumor necrosis factor - α（ TNF - α） in treatment of pediatric patients with moderate asthma. Methods A total of 64 pediatric patients with moderate asthma were randomly divided into group A （only received salmeterol fluticasone） and group B （treated with mon- telukast combined with salmeterol fluticasone）, and 33 healthy children were allocated in group C （control group）. Results The total effective rates of patients in group A and group B were 72.7% （24/33） and 93.9% （32/33） respectively. FEV1/FVC and PEF% in patients of group A and group B were significantly increased after treatment, and the increment in patients of group B was more significantly higher than that of group A （ P 〈 0.05 ）. The serum levels of IL - 13 and TNF - α in patients of group A and group B were significantly higher than those of controls in group C. The serum levels of IL - 13 and TNF - α in group A and group B after treatment for 12 weeks were significantly reduced in comparison with those before treatment, and their levels in group B were more significantly reduced than those of group A （ P 〈 0. 05 ）. Conclusion Monte- lukast combined with salmeterol fluticasone can significantly improve the lung function and inflammatory status in pediatric patients with moderate asthma, and they were more effective than single application of salmeterol fluticasone, hence it is worthy to be recommended for clinical applica- tion.
