代谢综合征老年男性患者骨代谢物和骨密度的关系研究

摘　　要：目的了解代谢综合征老年男性患者的骨代谢物和骨密度的关系。方法对照组210例、代谢综合征组378例、代谢综合征合并骨质疏松组80例、代谢综合征合并骨量减少组121例。比较各组的血压、血糖、血脂、骨代谢指标和骨密度水平。结果代谢综合征组、代谢综合征合并骨质疏松组、代谢综合征合并骨量减少组的骨密度值、骨密度T值和Z值均显著低于对照组(P均<0.05),骨钙素(OCN)、甲状旁腺素(PTH)、总-Ⅰ型胶原氨基端延长肽(tP1NP)、β-胶原特殊序列(β-CTx)均显著高于对照组(P均<0.05),VitD3显著低于对照组(P<0.05)。代谢综合征合并骨质疏松组的OCN与骨密度值呈正相关性(r=0.269,P<0.05),PTH与骨密度值呈负相关性(r=-0.244,P<0.05);TC、SBP、DBP与骨密度值呈负相关性(r=-0.195、-2.163、-2.037,P均<0.05)。结论骨代谢指标可作为代谢综合征老年男性患者骨量减少和骨质疏松的早期指标。Objective To explore the relevance between bone metabolic marker and bone mineral density in elderly male patients with metabolic syndrome. Methods The control group included 210 normal individuals, metabolic syndrome group included 378 cases metabolic syndrome, metabolic syndrome ＋ osteoporosis group included 80 cases of metabolic syndrome combined with osteoporosis, and metabolic syndrome ＋ bone mass reduction group included 121 cases of meta- bolic syndrome combined with bone mass reduction. The blood pressure, blood sugar, blood lipids, bone metabolic marker and bone mineral density were compared among the four groups. Results Bone mineral density, T-score and Z values, and levels of VitD3 of the three metabolic syndrome groups were significantly lower than those of the control group（ P 〈 0. 05 ） , but the levels of OCN, PTH, tP1NP, β-CTX were all higher than those of the control group （ P 〈 0.05 ）. The level of OCN was positively correlated with bone mineral density in the metabolic syndrome ＋ osteoporosis group （ r = 0. 269, P 〈 0.05 ） ;PTH,TC, SBP and DBP were negatively correlated with bone mineral density in the metabolic syndrome ＋ osteo- porosis group（ r = - 0. 244, - 0. 195, - 2. 163 and - 2.03, respectively, P 〈 0.05 ）. Conclusion The bone metabolic marker can be used as the early indicators of osteopenia and osteoporosis in elderly male patients with metabolic syn- drome.
