人工股骨头置换治疗老年股骨转子间骨折的19例临床分析

摘　　要：目的：探讨老年股骨转子间骨折患者采用人工股骨头置换术治疗的临床疗效。方法选取2010年2月至2013年4月入我院治疗的38例老年股骨转子间骨折患者为研究对象，按收治时间分为对照组和观察组，每组各19例。对照组采用动力髋螺钉内固定术治疗，观察组采用人工股骨头置换术治疗，比较两组患者治疗后的实际疗效。结果观察组的手术出血量、手术时间以及术后卧床时间均明显少于对照组，组间比较差异明显，具有统计学意义（P ＜0.05）。经术后回访，观察组术后1个月和3个月 Harris 评分均明显高于对照组，并发症发生率也明显低于对照组，均具有统计学意义（P ＜0.05）。术后6个月时该项评分无统计学意义（P ＞0.05）。结论老年股骨转子间骨折患者采用人工股骨头置换术进行治疗时手术出血量较少，手术时间和术后卧床时间较短，同时并发症的发生率也较低，值得临床借鉴和推广。Objective to investigate the elderly patients with femoral fracture between the rotor adopts the clinical curative effect of artificial femoral head replacement therapy.Methods from february 2010 to april 2013, works in our treatment of 38 cases of elderly patients with femoral fractures between rotor as the research object, according to the time were divided into control group and observation group, 19 cases in each group.The control group treated with dynamic hip screw fixation, the observation group was treated with artificial femoral head replacement, comparing the actual efficacy of two groups of patients after treatment.Results The observation group of surgery, blood loss, operative time, postoperative bed time were significantly less than the control group, comparing differences between groups, statistically significant （P 〈 0.05）.after review, observation group 1 month and 3 months after Harris scores were significantly higher than that of control group, incidence of complications and significantly lower than the control group, were statistically significant （P 〈 0.05）.There was no statistical significance in 6 months when the score （P〉0.05）.Conclusion Elderly patients with femoral fractures between rotor using artificial femoral head replacement for treatment surgery less blood loss, shorter operation time and postoperative bed time, at the same time, the incidence of complications is low, is worthy of reference for clinical and promotion.
