结核性脑膜脑炎25例的MRI表现

摘　　要：目的分析结核性脑膜脑炎25例的MRI影像表现,以提高本病的诊断率。方法收集经临床综合诊断的结核性脑膜脑炎25例的MRI资料,所有患者均行头颅MRI检查,其中20例同时进行头颅MRI平扫和增强。结果头颅MRI检查示20例患者脑内见粟粒性结核瘤,1例见大结核瘤及小血肿形成,增强扫描结核瘤结节样及环形强化;20例脑膜增厚、强化;5例脑底池狭窄;3例表现双侧基底节区对称性脑梗死样改变;2例幕上脑室积水。结论结核性脑膜脑炎MRI表现多样,结核瘤是其特征性表现。Objective To analyze the MRI manifestations of tuberculosis meningoencephalitis in order to im- prove accurate rate of diagnosis of this disease. Methods To collect MRI data of 25 cases tuberculosis meningoen- cephalitis diagnosed by clinical. All of the patients had the brain routine MRI scan, in which there were 20 cases with MRI brain contrast- enhanced scan. Results The brain MRI examination showed that there are some miliary tuberculosis lesions at the cerebral in 20 cases, tuberculoma and small hematoma in 1 case, and the tuberculomas were focal nodular and ring enhanced with the enhanced scanning; the meningeal thickened and enhanced in 20 cases; the base of the brain cistern narrowed in 5 cases; the supmtentorial hydrocephalus in 2 cases; the changes of cerebral infarction symmetrically located at the basal ganglia in 3 cases. Conclusion The MRI manifestations of tuberculosis meningoencephalitis are multiple, tuberculoma is its characteristic manifestation.
