犬前列腺组织弹性改变与膀胱出口梗阻严重程度的相关性

摘  要:目的评价犬良性前列腺增生(BPH)发展过程中前列腺组织弹性改变及其与膀胱出口梗阻(BOO)严重程度的相关性。方法雄性比格犬10只,通过去势手术、前列腺周围包绕两层尼龙网膜及雌雄激素联合应用3个月建立犬BPH致BOO模型。激素给予前、给予后1、2、3个月时,分别行经直肠超声前列腺检查。在建模手术前、手术后2周(给予激素前)、激素给予后1、2、3个月时分别对实验动物进行尿动力检查。比较前列腺体积、前列腺组织弹性模量及尿动力各指标在不同时间点的差异性,评价前列腺组织弹性模量与尿动力各指标的相关性。结果激素给予前后前列腺体积、弹性模量差异均有统计学意义(P均<0.01)。术前与术后2周尿动力各指标差异均无统计学意义(P均>0.05)。激素给予前后尿动力各指标差异均有统计学意义(P均<0.01)。前列腺组织平均弹性模量与尿动力各指标均具相关性,其中前列腺组织平均弹性模量与最大尿流率(Qmax,r=-0.80,P<0.01)及最大尿流率时逼尿肌压(Pdet@Qmax,r=0.76,P<0.01)的相关性更高。结论伴随BPH发展,犬前列腺组织弹性模量逐渐增高,其与BOO严重程度具有明显相关性。Objective To explore changes of prostatic elasticity during the development of benign prostatic hyperplasia (BPH) and the correlation with degrees of bladder outlet obstruction (BOO) in canine models. Methods Ten male beagle dogs were selected to establish canine models of BOO caused by BPH. Castration surgery was performed, the prostate was encapsulated with a double layer of nylon mesh, and then a combination of steroids was given for 3 months. Transrectal ul- trasound was performed pre- and 1, 2 and 3 months after hormone administration. Urodynamic evaluation was performed be{ore surgery, two weeks after surgery and 1, 2 and 3 months after hormone administration. Prostatic volume, Young modulus of prostate tissue and urodynamic parameters were compared at each time instance, and the correlation between Young modulus of prostate tissue and urodynamic parameters were evaluated. Results Prostatic volume, Young modulus of prostatic tissue were significantly different before and 1, 2 and 3 months after hormone administration (all P^0.01), while changes of urodynamic parameters were not significantly different before and two weeks after surgery (all P^0.05). Urodynamic parameters had significant differences before and after hormone administration (all P%0.01). Young modulus of prostatic tissue showed very significant correlation with urodynamic parameters, among them the maximum urinary flow rate (Qmax, r^-0.80, P^0.01) and detrusor pressure at maximum urinary flow rate (Pdet@Qmax, r=0. 76, P% 0.01) had higher correlation coefficient. ConelHsion Young modulus of prostatic tissue increases during the development of BPH. Significant correlation exists between Young modulus of prostatic tissue and degree of BOO in canine models.
