摘 要:目的观察静脉应用艾司洛尔控制心率对Double-Flash模式在CT冠状动脉造影(CTCA)中的应用率的影响。方法对心率≤100次/分的258例受检者行CTCA检查。将受检者随机分为两组,A组为自然心率组;B组为心率控制组,对其中心率>65次/分者静脉推注艾司洛尔注射液50mg。CTCA中对心率≤65次/分者应用Double-Flash模式扫描,对>65次/分用回顾性心电门控模式扫描。分别记录A、B两组中两种扫描模式的使用例数、图像质量、辐射剂量及B组药物不良反应发生率。结果 Double-Flash扫描模式A组应用率为61.33%(92/150),B组为98.15%(106/108),差异有统计学意义(P<0.05)。A组有效辐射剂量(ED)和CT容积剂量指数(CTDIvol)分别为(3.54±0.85)mSv和(13.94±0.65)mGy,B组分别为(1.81±0.95)mSv和(3.07±0.62)mGy,差异均有统计学意义(P均<0.05)。B组中无发生药物使用不良反应者。A组98.67%(148/150)、B组96.30%(104/108)图像可诊断,差异无统计学意义(P>0.05)。结论静脉应用艾司洛尔可安全有效地提高CTCA中Double-Flash扫描模式的应用率。Objective To observe the impact of intravenous administration of esmolol for heart rate control on application of Double-Flash mode in CT coronary angiography (CTCA). Methods Totally 258 subjects with heart rate (HR) no more than 100 bpm underwent CTCA. The subjects were randomly divided into two groups. Subjects in group A had natural HR, while in group B (HR control group) were intravenously given 50 mg esmolol while HR〉65 bpm. Double-Flash mode (HR≤65 bpm) and retrospective ECG gating mode (HR〉65 bpm) were applied during CTCA. The application rate of Double-Flash mode, image quality, volume CT dose index (CTDIvol) and effective radiation dose (ED) were recorded, respectively. Results In group A, the application rate of Double-Flash mode was 61.33% (92/150), which in group B was 98.15% (106/108, P〈0.05). ED of group A was (3.54±0.85)mSv, while in group B was (1.81±0.95)mSv, CT- DIvol of group A was (13.94±0.65)mGy, while in group B was (3.07±0.62)mGy (both P〈0. 05). No adverse inci- dence of esmolol was found in group B. There were 98.67% (148/150) images in group A and 96.30% (148/150) in group B qualified for diagnosis (P〉0.05). Conclusion Intravenous administration of esmolol may increase the application rate of Double-Flash mode in CTCA effectively and Safely.
