摘　　要：目的 研究基于CRP和ESR计算RA患者DAS28在评价患者疾病活动度方面的差别.方法 入选222例RA患者,记录患者的性别、年龄、病史、肿胀关节数、压痛关节数、CRP、ESR、视觉模拟评分.分别计算DAS28-ESR和DAS28-CRP.统计学方法采用t检验、Pearson相关分析.结果 DAS28-ESR和DAS28-CRP具有良好的线性相关关系（r=0.968,P＜0.05）.DAS28-CRP的分布（3.3±1.7）较DAS28-ESR的分布（3.9±1.8）偏左侧,差异有统计学意义（P＜0.05）.女性患者DAS28-CRP和DAS28-ESR的差异（0.59±0.43）较男性患者（0.24±0.45）更大,差异有统计学意义（P＜0.05）.DAS28-CRP和DAS28-ESR之间的差异与病史、年龄均无相关性（P＞0.05）.结论 DAS28-CRP和DAS28-ESR在评价RA患者疾病活动度、制定达标治疗方案时存在差异,值得重视.Objective To compare the disease activity score （DSA） 28-CRP and DAS28-ESR in patients with rheumatoid arthritis.Methods Two hundred and twenty-two patients were enrolled,and their sex,age,disease duration,swollen joint count,tender joint count,CRP,ESR,visual analogue scale were recorded.DAS28-ESR and DAS28-CRP were calculated and then analyzed by t test and Pearson＆#39;s correlation test.Results There was a significant linear correlation between DAS28-ESR and DAS28-CRP （P〈0.05）,with correlation coefficient of 0.968.Both DAS28-CRP （3.3±1.7） and DAS28-ESR （3.9±1.8） scores presented with normal distribution （P〉0.05）,with the peak of the DAS28-CRP left to that of the DAS28-ESR.There was statistically significant difference between these two （P〈0.05）.The difference between DAS28-CRP and DAS28-ESR was much higher in female （0.59±0.43） than in male （0.24±0.45,P〈0.05）.The difference between DAS28-CRP and DAS28-ESR was not related to age and disease duration.Conclusion Attention should be paid to the assessment score when making the plan of ＆quot;treating to target＆quot; since there is difference between DAS-28-ESR and DAS-28-CRP.
