乳腺癌三维超声定性及定量分析与Ki-67的相关性研究

摘　　要：目的探讨三维彩色直方图血流定量参数、三维超声图像特征与Ki-67表达间的相关性。方法分析75例76个经手术病理确诊的乳腺癌肿块三维超声特征、组织学分级特点,采用三维彩色直方图技术测量体积(volume,V)、血管指数(vascularization index,VI)、血流指数(flow index,FI)、血管血流指数(vascularization-flow index,VFI),并与Ki-67表达情况对照,分析它们之间的相关性。结果 Ki-67阳性组较阴性组肿块内VI、VFI测值明显增加(P<0.05)。Ki-67阳性组V、FI测值均值较阴性组大,但差异没有统计学意义(P>0.05)。Ki-67阳性组更容易出现淋巴结转移(P<0.05)。Ki-67阳性表达与组织学分级呈正相关(P<0.05)。"太阳征"在Ki-67阳性组与阴性组间表达未见明显差异(P>0.05)。结论乳腺癌三维超声定性及定量指标与Ki-67阳性表达之间存在一定的相关性。三维彩色直方图定量分析能更精确评价乳腺肿块内部的血供情况,对乳腺癌生物学行为和预后判定有重要意义。Objective To investigate the relationship between Three-dimensional sonography qualitative and quantitative features and the expression of Ki-67 in Breast cancers, using Color Histogram techniques. Methods Anal- ysis the characteristic of 3D sonography in 75 breast cancer patients confirmed by pathology, using three-dimensional color Histogram techniques. Compared the correlation of sonography features, V, VI, FI, VFI, histological grade, and the expression of Ki-67. Results The indices of VI,VFI were significantly higher in the tumors with Ki 67 positive expression compared to those with Ki-67 negative expression （P〈0.05）. The correlation of lymphatic metastasis with expression of Ki-67 was significantly positive（P〈0.05）. The expression of Ki-67 was positively correlated with the histological stage（P〈0. 05）. The mean value of V,FI were higher in the positive group, but the difference was not statistically significant （P〈0.05）. The ＂sun-like symptom＂ of breast cancer was not related to the expression of Ki-67 （P〈0.05）. Conclusions Qualitative and quantitative features of three-dimensional sonography were related to the ex- pression of Ki-67 in breast cancer in some extent. Color Histogram techniques can give more accurate information about the blood supply of breast cancer. So the combination of them is important for the determination of biological be- havior and prognosis in breast cancer.
