摘  要:目的探讨三维斑点追踪(3D-STE)技术评价冠心病患者经皮冠状动脉介入治疗(PCI)术短期疗效的临床应用价值。方法应用3D-STE技术测量并比较45例冠心病和45例对照组左室整体及16个节段的心肌收缩期峰值纵向、圆周及径向应变,并于PCI术后1周、1个月复查上述指标的改变。结果 PCI术前,冠心病组左室整体各向及8个纵向、10个圆周、12个径向节段的应变值较对照组显著降低(P<0.05);PCI术后1周、1个月上述应变值较术前显著改善(P<0.05),呈递增趋势,但仍显著低于对照组(P<0.05)。结论 3D-STE技术能快速直观地定量分析冠心病患者的节段性室壁运动异常,PCI术后冠心病患者左室整体及缺血节段应变值较PCI术前均有所改善,3D-STE可应用于PCI术短期疗效的评价。Objective To evaluate left ventricular(LV) systolic function in patients with coronary heart disease (CHD) after percutaneous coronary intervention(PCI) with 3D speckle tracking echocardiography (3D-STE). Methods Forty-five patients with CHD and another 45 patients without CHD were enrolled in this study. LV global and seg- mental longitudinal, circumferential and radial strains were measured in all subjects before PCI by 3D-STE and com- pared between groups. Changes in LV global and segmental strains were reevaluated one week and one month after PCI. Results Compared with the control group, LV global and 8 longitudinal, 10 circumferential and 12 radial seg- mental strains were significantly decreased in the CHD group before PCI (P〈0.05). LV global and ischemic segmen- tal strain values were significantly increased one week and one month after PCI (P〈0.05) but were still significantly lower than the control group(P〈0.05). Conelusions 3D-STE can quickly and quantitatively detect LV segmental wall motion abnormalities in patients with CHD, LV ischemic segmental and global strains improvements after PCI. 3D-STE may be applied to evaluate the short-term efficacy of PCI.
