摘 要:目的研究脉冲式治疗超声在不同峰值负压情况下,激励产生的微泡超声空化与兔VX_2肿瘤微循环阻断程度的初步关系。方法 30个种植于兔肌肉内的VX_2肿瘤按照峰值负压随机分为3组,分别为2 184 kPa、1 785 kPa组及1 019 kPa组,进行超声联合微泡治疗。各组肿瘤治疗前后分别行超声造影定量分析。最后获取肿瘤行病理检查。结果治疗后2 184 kPa组肿瘤血流灌注基本消失,1 785 kPa组血流灌注显著下降,但1 019 kPa组血流灌注视觉强度却有少许增加。2 184 kPa组及1 785 kPa组肿瘤治疗后超声造影峰值强度(P1)及曲线下面积(AUC)显著低于治疗前(P<0.01);1 019 kPa组治疗前后比较无显著差异。病理检查见各组肿瘤微血管壁断裂出血,内皮细胞肿胀,组织水肿。结论峰值负压在1 785 kPa和2 184 kPa水平时超声空化可阻断VX_2肿瘤微循环,且峰值负压越大,阻断效果越显著。Objective To investigate the relationship between microbubble enhanced ultrasound (MEUS) excited by different peak negative pressures (PNP) and VX2 tumor blood perfusion cessation. Methods 30 intramuscular VX2 tumors were assigned to three groups treated by 2 184 kPa, 1 785 kPa, and 1 019 kPa PNPs respectively. The tumor blood perfusion was quantified using contrast-enhanced ultrasound (CEUS) before and after treatment. Finally, the tumors were harvested for histological examination. Results CEUS shows that the tumor perfusion was almost van- ished when treated by MEUS at 2184 kPa PNP and was significantly declined when treated by MEUS at 1 785 kPa PNP. However, the tumor perfusion slightly increased after the treatment of 1 019 kPa PNP. The peak intensitity (PI) and area under curve (AUC)also decreased significantly after the treatment of 2 184 kPa and 1 785 kPa (P~ 0.01). There is no significant difference in the 1 019 kPa group. The pathological observed the bleeding of tumor cap- illary wall rupture, the swelling of endothelial eells and the tissue edema. Conclusions The blood perfusion of VX2 tumor can be arrested under the MEUS treatments at 1 785 kPa and 2 184 kPa with microbubble. The blocking effect of tumor microcireulation inereased with the PNP addition.
