摘 要:目的探讨载脂蛋白B基因XbaI、EcoRI位点稀有等位基因与内蒙古中西部地区汉族和蒙古族胆石症人群的相关性。方法收集2010年4—10月包头医学院第一附属医院100例胆石症患者(结石组)及同期行健康体检的115例正常人(对照组)的临床资料,采用病例对照研究方法和聚合酶链反应一限制性片断长度多态性(PCR—RFLP)技术,检测和分析内蒙古中西部地区的汉族和蒙古族结石组和对照组血样的载脂蛋白B基因XbaI、EcoRI多态性,包括XbaI位点的x±x±、x±X-、X—X一基因型及等位基因x-、x±(稀有等位基因);EcoRI位点的E±E±、E±E-、E—E一基因型及等位基因E±、E-(稀有等位基因)。检测各组的血脂水平,包括TG、TC、HDL和LDL。计数资料行疋。检验,计量资料行t检验。结果汉族人群与蒙古族人群均无X±x±基因型,且蒙古族人群中亦未发现稀有等位基因X±和E-的存在。在汉族人群中,结石组LDL为(2.8±0.9)mmol/L,显著高于对照组的(1.9±0.8)mmol/L(t=2.800,P〈0.05);蒙古族人群中,结石组HDL、LDL分别为(1.74-0.3)mmol/L、(3.5±0.8)mmol/L,显著高于对照组的(1.2±0.3)mmol/L、(2.8±0.9)mmol/L(t:7.596,2.549,P〈0.05)。蒙古族人群中,结石组TG、TC、HDL、LDL分别为(3.1±1.6)mmol/L、(5.6±1.0)mmol/L、(1.7±0.3)mmol/L、(3.5±0.8)mmol/L,均显著高于汉族结石组的(1.2±0.6)mmol/L、(4.4±1.2)mmol/L、(1.3±0.3)mmol/L、(2.8±0.9)mmol/L(t=5.501,3.667,4.448,3.430,P〈0.05)。蒙古族对照组TG、TC、LDL分别为(2.6±1.7)mmol/L、(5.1±1.1)mmol/L、(2.8±0.9)mmol/L,均显著高于汉族对照组的(1.3±0.7)mmol/L、(3.9±0.9)mmol/L、(1.9±0.8)mmol/L(t=4.298,4.772,3.888,P〈0.05)Objective To investigate the relationship between the apolipoprotein B (ApoB) gene Xba I and EcoR I polymorphisms and eholelithiasis in Han and Mongolian population in the Midwest Area of Inner Mon- golia. Methods The clinical data of 100 patients with eholelithiasis and 115 healthy individuals at the First Affil- iated Hospital of Medical College of Baotou from April to October in 2010 were collected. A ease-control study which detected ApoB alleles of patients with cholelithiasis ( eholelithiasis group) and healthy individuals ( control group) in Han nationality and Mongolian nationality in the Midwest Area of Inner Mongolia was conducted by poly- merase chain reaction-restriction fragment length polymorphism, which included Xba I ( X + X + , X + X - , X - X - , X + , X - ) and EcoR I ( E + E +, E - E -, E + E -, E +, E - ). The serum lipid ( including triglycer- ide, total cholesterol, high density lipoprotein and low density lipoprotein) levels in different groups were detected. The count data and the measurement data were analyzed using the chi-square test and t test, respectively. Results C, enotype X + X + was not found in the Han and Mongolian population, and Xba I ( X + ) or EcoR I ( E - ) alleles was not found in the Mongolian population. The levels of low density lipoprotein were ( 2.8 ±0.9 ) mmol/Lin the cholelithiasis group, which was significantly higher than ( 1.9±0.8 ) mmol/L of the control group in the Han population (t = 2. 800, P 〈 0.05 ). The levels of high density lipoprotein and low density lipoprotein were ( 1.7 + 0.3 ) mmol/L and ( 3.5 + O. 8 ) mmol/L of the choMithiasis group, which were significantly higher than ( 1.2 ~ 0.3 ) mmol/L and ( 2.8 ± 0.9 ) mmol/L of the control group in the Mongolian population ( t = 7. 596, 2. 549, P 〈0.05).
