TLIF手术方法配合补阳还五汤治疗腰椎退行性疾病临床研究

摘　　要：目的：观察TLIF手术方法配合补阳还五汤治疗腰椎退行性疾病的临床疗效。方法：将60例患者按随机数字表法分为对照组和治疗组。治疗组采用TLIF手术方法配合补阳还五汤加减口服治疗，对照组TLIF手术方法治配合塞来昔布胶囊口服治疗。治疗后随访时间6个月一1a，用日本骨科学会下腰痛评分系统（JOA评分）对手术前、手术后两周进行疗效评价比较。结果：手术前后JOA评分比较有显著性差异，患者腰痛和下肢放射性疼痛等临床症状均有明显缓解。60例患者平均住院天数为13．4d，出血量平均为131mL，随访结束后正侧位x线片显示：60个椎间植骨全部融合，融合率100％，平均融合时间为7个月，均无并发症。结论：TLIF手术方法损伤小，出血少，并发症少，复发率低。早期配合补阳还五汤治疗可以有效改善临床症状．促进伤口愈合。Objective： To observe the clinical curative effect of TLIF surgical method and buyang huanwu deCOction in treating lumbar degenerative disease. Methods： 60 patients were randomly divided into the control group and the treatment group. The treatment group was given TLIF surgical method and buyang huanwu decoction in treating lumbar degenerative disease, and the control group was given TLIF surgical method and celecoxib capsules oral administration. Follow-up 6 months - 1 a after treatment, evaluated and compared cur- ative effect of patients preoperative and postoperative two weeks with the back pain score system by Japanese orthopaedic association （JOA score） . Results： The difference of JOA score before and after surgery was significant, and back pain and lower limb radiation pain and other clinical symptoms of patients were significantly alleviated. The average hospitalization days of 60 patients was 13.4 days, and an average of amount of bleeding was 131 mL. After follow-up, positive-lateral X-ray showed： all 60 intervertebral bone graft fusion, and the fusion rate was 100% , average convergence times for seven months, and there were no complications. Conclusion：TLIF surgery method has less injury,less bleeding, fewer complications and lower recurrence. Early cooperation with buyang huanwu decoction can effectively improve the clinical symptoms, promote wound healing.
