重组人酸性成纤维细胞生长因子在半月板损伤关节镜术中的临床应用

摘　　要：目的评价重组人酸性成纤维细胞生长因子(rh-aFGF)冲洗关节腔对半月板损伤的修复作用。方法入选我国3个中心480例半月板损伤患者,根据随机数列分为对照组和治疗组,两组根据撕裂类型给予关节镜修整、切除术,清理关节腔治疗。对照组240例,339条患肢,给予生理盐水冲洗关节腔;治疗组240例,324条患肢,给予rh-aFGF溶于生理盐水冲洗关节腔。术后随访6个月,观察关节功能的恢复情况。结果随访6个月,治疗组关节功能优等率为41.7%,对照组为33.4%,两组差异有统计学意义(χ2=4.422,P=0.035)。治疗组纵裂、水平裂、斜裂的Lysholm膝关节功能评分分别为(98.7±5.2)、(95.2±5.4)、(89.3±2.3)分,对照组分别为(81.6±5.0)、(78.4±7.4)、(76.6±6.4)分,两组比较差异有统计学意义(P<0.05)。结论关节腔内冲洗rh-aFGF,有利于半月板损伤修复,改善关节功能。Objective To study the efficacy of recombinant human acidic fibroblast growth factor in promoting menisci injury healing. Methods 480 patients of meniscus injury from 3 research centers were selected and divided into treat- ment group and control group according to the random number. The two groups were treated with arthroscopy trimming and resection, articular cavity clean according to the tear type. 240 cases （339 limbs） in control group were treated with joint cavity physiological saline flush; 240 cases （324 limbs） in treatment group were treated with joint cavity rh-aFGF flush. The recovery of joint function between the two groups were observed after 6 months＂ post-operation following-up. Results The excellent grade rate of treatment group were 41.7%, and the excellent grade rate of control group were 33.4%. the difference was statistically significant （X2 = 4.422, P = 0.035）. The Lysholm grade of diastema, fissura hori- zontalis, oblique fissure were （98.7±5.2）, （95.2±5.4）, （89.3±2.3） scores, and the control group were （81.6±5.0）, （78.4± 7.4）, （76.6±6.4） scores respectively, the differences were statistically significant （P 〈 0.05）. Conclusion The joint cavity rh-aFGF flush can repair the menisci injury and improve joint function.
