三乙醇胺乳膏在微创腋臭术后的应用

摘　　要：目的观察三乙醇胺乳膏对微创腋臭切除术后伤口愈合的临床效果。方法选择2009年6月～2013年6月深圳市人民医院一门诊部普外科进行微创腋臭手术的患者68例，随机分为治疗组和对照组，每组各34例。治疗组在缝合伤口后及每次换药中均使用三乙醇胺乳膏，对照组则使用凡士林纱布覆盖。观察皮瓣局部溃疡或坏死等术后并发症发生情况、创面愈合时间，并随访术后6个月切口瘢痕情况。结果①治疗组未出现皮瓣局部溃疡或坏死等情况，对照组6例出现皮瓣局部溃疡或缺血性坏死；对照组并发症发生率明显高于治疗组，差异有统计学意义（P〈0．05）。②治疗组平均伤口愈合时间[（7．15±1．98）d]显著低于对照组[（11．76±3．43）d]，差异有统计学意义（P〈0．05）。⑧治疗组切口瘢痕质软、细小平整；对照组切口瘢痕多呈红褐色、质韧隆起。治疗组切口瘢痕平均宽度[（0．55±0．23）mm]显著低于对照组[（1．52±0．66）mm]，差异有统计学意义（P〈0．05）。结论在微创腋臭切除术后，使用三乙醇胺可有效缩短伤口愈合时间及降低并发症的发生，同时减轻远期瘢痕增生反应。Objective To observe the clinical effect of Trolamine Cream on the wound after bromidrosis minimally in- vasive surgery. Methods From June 2009 to June 2013, 68 cases of patients with the wound after bromidrosis minimal- ly invasive surgery were selected and randomly divided into treatment group and control group with 34 cases in each group. The treatment group was treated with Trolamine Cream on the surface of flap and wound. While the control group was treated with vaseline gauze. The postoperative complications such as flap local ulcer or necrosis, wound heal- ing time in two groups were observed; the incision scar 6 months after the operation was followed-up. Results ①No flap ulcer or necrosis was found in the treatment group and 6 cases with flap ulcer or necrosis was found in the control group; the complication occurrence rate in treatment group was higher than that in control group, the difference was statistically significant （P 〈 0.05） ②The average wound healing time in treatment group [（7.15±1.98） d] was lower than that in control group [（11.76±3.43） d], the difference was statistically significant （P 〈 0.05）.③The scar in treatment group was soft and flat; and the sear in control group was bronzing, hard and swollen. The average width of the ineision sear in treatment group [（0.55±0.23） ram] was lower than that in control group [（1.52±0.66） ram], the difference was sta- tistically significant （P 〈 0.05）. Conclusion After the minimally invasive armpits resection, Trolamine Cream can sig- nifieantly shorten the wound healing time and reduce occurrence of eomplieations, also can ease the forward sear prolif- eration response.
