摘　　要：目的探讨经闭孔无张力阴道吊带悬吊术（TVT-O）治疗老年女性压力性尿失禁的有效性、安全性及其并发症的防治。方法对2009—2011年93例中重度老年女性压力性尿失禁患者采用TVT-O治疗，手术后采用Grouts-Blaivas评分法评价治疗效果，并定期随访观察远期疗效。结果所有手术过程均顺利完成，均未出现膀胱损伤或穿孔、吊带排斥和闭孔神经血管的损伤等严重并发症。患者术后随访6～23月，按Grouts．Blaivas评分法，尿失禁的最终治愈率为91％（85／93），改善良好7例，改善中等1例，无失败病例。结论TVT-O手术治疗老年女性压力性尿失禁，操作简便，手术时间短，并发症少，疗效满意，能有效提高老年女性患者的生活质量。Objective To assess the effectiveness and safety of tension-free vaginal tape-obturator technique （TVT- O） for stress urinary incontinence in old female. Methods Ninety-three patients with moderate or severe stress urinary in- continence undergoing TVT-O from 2009 to 2011 were followed for 6 to 23 months. The outcome effect was evaluated according to the classification of Grouts-Blaivas outcome score. Results TVT-O had favorable results in all 93 patients without any operative failure or severe complication. The cure rate was 91% （85/93）, and the other eight patients also got improvement. Conclusions TVT-O is easy to carry out,with few complications and improved short term cure rate and quality of life.
