老年慢性心力衰竭患者血浆尾加压素Ⅱ与尿水通道蛋白-2关系的研究

摘　　要：目的测定老年慢性心力衰竭(CHF)患者的血浆尾加压素Ⅱ(U-Ⅱ)及尿水通道蛋白-2(AQP2)水平并观察其与CHF的相关性,探讨它们对老年CHF的临床诊疗价值。方法 2012年1月至2013年6月住院的202例老年CHF患者,按NYHA心功能分级标准分为心功能Ⅱ级56例,心功能Ⅲ级69例和心功能Ⅳ级77例,同时设健康对照组100例,用酶联免疫法检测血浆中U-Ⅱ和尿液中AQP2浓度,比较其变化。结果 CHF患者血浆U-Ⅱ含量明显低于对照组(P<0.05),CHF各组U-Ⅱ值逐步下降,U-Ⅱ与CHF心功能分级呈负相关(P<0.05)。CHF组患者尿液AQP2水平较正常组明显上升(P<0.05),CHF各组AQP2值逐步上升,AQP2与CHF心功能分级呈正相关(P<0.05)。U-Ⅱ水平与AQP2水平呈负相关(P<0.05)。结论 U-Ⅱ和AQP2水平能反映CHF严重程度,U-Ⅱ可能影响CHF患者肾脏的水钠代谢,成为心力衰竭治疗新靶点。Objective To investigate the plasma level of urotensin II （U-11 ） and the urine level of aquaporin 2 （AQP2） in the elderly patients with chronic heart failure（CHF） and the relationship of U-Ⅱ and AQP2 with CHF, and to discuss the clinical values of U-I1 and AQP2 in the diagnosis of CHF. Methods 202 hospitalized CHF patients from January 2012 to June 2013 were analyzed. According to the classification standard of NYHA, there were 56 cases of grade Ⅱ , 69 of grade Ⅲ and 77 of grade IV. Meanwhile, 100 healthy controls were enrolled. The level of U- Ⅱ and AQP2 were tested by ELISA kits and compared between 2 groups. Results Plasma U- Ⅱ in CHF patients was significantly lower than that in healthy controls（P〈O. 05）, and U-I1 was negatively correlated with heart function classification（ P〈O. 05）. Urine AQP2 was obviously increased in CHF patients, and AQP2 was positively correlated with heart function classification（ P〈 O. 05 ）. Plasma U- Ⅱ was negatively correlated with urine AQP2（P〈0.05）. Conclusions The levels of plasma U- Ⅱ and urine AQP2 could show the severity of CHF. U-Ⅱ may influence the sodium-water metabolism of kidney and may be the new therapeutic target for CHF.
