摘 要:目的:对血清CA125检测联合PET/CT显像在诊断早期卵巢癌中的应用于临床价值进行分析和探讨。方法:取卵巢病变病人110例为本次研究对象,对其实施血清CA125检测联合全身18F-FDG PET/CT显像。结果:在本组病人中,70例为卵巢癌,剩余40例属良性病变。在血清CA125指数水平方面,卵巢良性病变病人与卵巢癌病人之间存在显著差异(P<0.05);而卵巢良性病变病人与卵巢癌病人在SUV最大值(SUVmax)方面同样存在显著差异(P<0.05);在早期诊断卵巢癌过程中,18F-FDG PET/CT显像灵敏度为95.1%,特异性为79.09%,阴性与阳性预测值分别为88.5%和93.1%,准确性为80.9%;在卵巢癌早期诊断效能方面,18FFDG PETCT显像在上述指标方面均高于血清CA125检测,但18F-FDG PET/CT显像与CA125检测联合可有效提高前者诊断准确性(88.3%)以及特异性(91.0%)。结论:在早期诊断卵巢癌过程中,18F-FDG PET/CT显像具有相对特异性以及高灵敏度,联合18F-FDG PET/CT显像与血清CA125检测有利于早期诊断卵巢癌准确性与特异性的提高。Objective:To analyze and discuss the application and clinical value of serum CA125 detection combined with PET/CT imaging in the diagnosis of early ovarian cancer. Methods:A total of 110 patients with ovarian lesions were taken as the subjects, who received serum CA125 detection combined with systemic 18 F-FDG PET/CT imaging. Results:Among the patients, 70 were ovarian cancer and the rest 40 were benign lesions. Patients with benign ovarian lesions and those with ovarian cancer showed significant difference(P&lt;0. 05)in both the serum CA125 level and SUVmax. In the early diagnosis of ovarian cancer,the sensitivity of 18F-FDG PET/CT imaging was 95. 1%,the specificity was 79. 09%,the negative and positive predictive values were 88. 5%and 93. 1%,and the accuracy was 80. 9%. As for the early diagnostic efficiency of ovarian cancer,the above indicators of 18 F-FDG PET/CT imaging were all higher than those of serum CA125 detection,but the combination of 18 F-FDGPET/CT imaging and CA125 detection can effectively improve the diagnostic accuracy of the former by 88. 3%and the specificity by 91. 0%. Conclusion:In the early diagnosis of ovarian cancer,18 F-FDG PET/CT imaging has relative specificity and high sensitivity. The combination of 18 F-FDG PET/CT imaging and serum CA125 detection can help to improve the accuracy and specificity of early ovarian cancer diagnosis.
