摘 要:目的应用三维斑点追踪技术检测2型糖尿病患者左心室三维位移及扭转。方法合并微血管病变的2型糖尿病患者33例(糖尿病组)和体检健康者30例(对照组),采集左心室全容积图像输入TOMTEC软件,计算左心室16节段三维位移,心尖部、基底部及左心室整体扭转角度。结果与对照组比较,糖尿病组前间隔、前壁、后壁基底部和中间部,侧壁中间部,下壁基底部、心尖部,后间隔中间部、心尖部三维位移均减低(P<0.05);糖尿病组心尖扭转角度和整体扭转角度较对照组减低(P<0.05)。结论三维斑点追踪技术可准确反映糖尿病患者左心室收缩功能的变化,是一种简单、省时、可靠的超声新技术,对评价糖尿病患者左心功能有较高应用价值。Objective To measure the left ventricular displacement and twist by three-dimensional speckle tracking echocardiography (3D-STE) in patients with type 2 diabetes mellitus (T2DM). Methods Thirty-three T2DM patients complicated with microangiopathy and 30 healthy volunteers were obtained real-time three-dimensional images of left ventricle and put into TomTec 4D software to calculate three-dimensional displacement of 16 myocardial segments, as well as twist of the apical, basal segments and global. Results Compared with healthy controls, three-dimensional displacement decreased significantly in the basal and middle parts of anteroseptal, anterior and posterior wall, the middle part of lateral wall, the basal and apical parts of inferior wall, the middle and apical parts of septum in T2DM patients (P〈0.05), and the peaks of left ventricular apical segment and global twist reduced (P〈0.05). Conclusions As a simple, time-saving and reliable technique, 3D-STE has a high value to the evaluation left ventricular function in T2DM patients.
