右美托咪定对食管癌根治术患者炎性因子的影响

摘　　要：目的观察右美托咪定对胸腔镜下行食管癌根治术患者肿瘤坏死因子-a（tumornecrosisfactor-a，TNF—n）、白细胞介素（interleukinIL）-6、IL-10的影响，分析右美托眯定对肺组织的保护作用。方法胸腔镜下行食管癌根治术患者40例，随机分为右美托咪定组和对照组各20例，右美托咪啶组麻醉后给予右美托咪定1tLg／kg（10min注射完毕），然后以0．5Mg／（kg·h）静脉滴注维持；对照组静脉注射等量生理盐水；分别于手术前（T0）和手术开始45rain（T1）、90min（T2）、术毕（T3）以及术后24h（T4）行血气分析，采用ELLSA法检测TNF-a、IL-6、IL10水平，并进行组间比较。结果与对照组比较，右美托咪啶组T1～T4时间点TNF—a、IL-6水平降低，IL-10水平升高，2组比较差异有统计学意义（P〈0．05）；右美托咪啶组pa（O2）在T1、T2、T3时间点与对照组比较差异有统计学意义（P〈O．05）。结论右美托咪定对胸腔镜下行食管癌根治术患者肺组织有保护作用。Objective To observe the protective effect of dexmedetomidine on lung tissue by observing the influence of dexmedetomidine on tumor necrosis factor-a （TNF-a）, interieukin-6 （IL-6） and interleukin-10 （IL-10） in patients undergoing radical operation for esophageal cancer. Methods Forty patients with esophageal cancer undergoing radical operation under thoracoseopy were randomly divided into dexmedetomidine group and control group, with 20 patients in each group. Dexmedetomidine group was intravenously injected dexmedetomidine （1 /μg/kg） in 10 minutes, followed by intravenous drip maintenance of 0.5 μg/（kg . h）. Control group was given the same volume of normal saline. Blood gas analysis was done before operation （To）, 45 minutes after the beginning of operation （T1）, 90 minutes after the beginning of operation （T2）, when the operation was finished （T3） and 24 hours after operation （T4）. The levels of TNF-a, IL-6 and IL-10 were detected by ELISA technique and compared between groups. Results Compared with control group, TNF a and IL-6 decreased and IL-10 increased significantly at every time point in dexmedetomidine group （P〈0.05）. There was significant difference in pa（O2 ） at the . Conclusions Dexmedetomidine has lung protective thoracoscooy for esophageal cancer. time points of T1 , T2 and T3 between two groups （P〈0.05 effect in patients undergoing radical operation underthoracoscopy for esophageal cancer.
