养老机构痴呆老人生活质量自评和他评的比较分析

摘　　要：目的比较养老机构痴呆老人生活质量自评和他评的差异,为评价痴呆老人的生活质量提供依据。方法从北京、广州两所社会养老机构选取85例轻、中度痴呆老人,采用老年痴呆患者生活质量量表对痴呆老人生活质量进行自我评定和照顾者他评,对自评和他评的结果进行比较分析。结果痴呆老人生活质量自评和他评总分及各维度得分之间差异均无统计学意义(P>0.05)。相关分析显示,除轻度痴呆老人在自尊维度上自评与他评得分无相关性外,痴呆老人生活质量自评和他评总分及其他各维度得分相关性较高(P<0.01或P<0.05)。结论养老机构轻、中度痴呆老人自评、他评生活质量相关程度较高,当痴呆老人无法完成生活质量自评时,其直接照顾者可考虑作为痴呆老人生活质量的代评人。Objective To compare the results of self-rating and caregiver-rating quality of life of dementia patients in elderly homes,and to provide basis for evaluating quality of life of dementia patients.Methods Totally,85 mild to moderate dementia patients from two social elderly homes in Beijing and Guangzhou were recruited.The quality of life of dementia patients were rated by themselves and their formal caregivers using the Dementia Quality of Life （DQoL） scale.The results of quality of life between self-rating and caregiver-rating were compared.Results There were no significant differences in the total score and each factor scores of quality of life between self-rating and caregiver-rating （P ＞ 0.05）.Correlation analysis showed that,except the dimension of self-esteem in mild dementia patients,positive correlations were found in the total score and the dimension scores of quality of life between self-rating and caregiver-rating （P ＜ 0.01 or P ＜ 0.05）.Conclusion The results of self-rating and caregiver-rating on the quality of life of mild to moderate dementia patients are highly correlated.It is suggested that when the dementia patients cannot rate their quality of life,the formal caregivers can act as the proxy to rate the quality of life.
