小切口皮下剥离刮除术治疗腋臭手术并发症分析及预防措施

摘　　要：目的探讨腋臭患者行小切口皮下剥离刮除术的治疗效果及并发症的预防与处理。方法对273例腋臭患者浸润肿胀麻醉下，通过1erfl切口皮下剥离刮除术清除腋窝皮下的皮脂腺、大汗腺及毛囊，术后放置引流并加压包扎，9天后拆线。结果273例患者手术均成功。10例术后出现并发症：3例在术后6～12h发生出血，形成血肿；1例出现部分皮瓣坏死：2例出现伤口裂开；1例局部形成瘢痕挛缩；1例单侧有异味残留；2例单侧复发。经及时有效的治疗后均痊愈。结论小切口皮下剥离刮除术治疗腋臭具有创伤小、切口隐蔽、外形美观、无功能障碍的特点。术中规范操作，采取相应的措施可以减少并发症的发生，使其对患者的损害减少到最小。Objective Evaluating the therapeutic effect of decollement and curettage with a mini-incision in the treatment of axillary osmidrosis. Analyzing the complications and providing the preventive measures. Methods Gather 273 cases of axillary osmidrosis treated by decollement and curettage with a mini-incision. The subcutaneous sebaceous glands, sweat glands and hair follicles was cleared through lcm long incision with infiltrating swelling anesthesia. Postoperative compression bandage and drain- age were placed. The stitches was taken out for 9 days. Results Surgery of all cases was successful. There were 10 cases of com- plications occurred, Including 3 cases of hemorrhage, the formation of hematoma, 2 cases with wound dehiscence, 1 case of scar contracture, 1 case of unilateral smell residue and 2 cases of unilateral relapse. All cases recovered after prompt effective treat- ment. Conclusion Decollement and curettage with a mini-lncision has the advantage of concealment, good appearance, no dys- function. Standardized operation and corresponding measures can reduce these complications.
