原发性高血压合并心力衰竭患者临床特点分析

摘　　要：目的分析原发性高血压(EH)合并心力衰竭的临床特点。方法选取昆明市第三人民医院2012年3月—2013年3月收治的EH合并心力衰竭患者60例作为A组,选择同期未合并心力衰竭的EH患者40例作为B组,比较两组患者动态血压(收缩压和舒张压)、超声心动图指标(室间隔和左心室舒张末内径)及实验室检查指标〔C反应蛋白(CRP)、肌酸激酶同工酶(CK-MB)及脑钠肽(BNP)〕。结果 A组患者收缩压、舒张压、室间隔、左心室舒张末内径、CK-MB、BNP及CRP水平均高于B组(P<0.05)。结论 EH合并心力衰竭患者与EH患者比较,其收缩压和舒张压均明显升高、室间隔肥厚、左心室舒张末容积增加,积极控制血压及纠正心力衰竭对患者预后具有重要意义。Objective To obsewe the characteristic of essential hypertension （EH） combined with heart failure. Methods 60 patients with EH combined with heart failure admitted to our hospital from March 2012 to March 2013 were selected as group A, 40 patients with EH at the same time were selected as group B. The ambulatory blood pressure （SBP and DBP）, echocardiographie indexes （sept＇,d and left ventricular end -diastolic diameter） , CRP, CK - MB and BNP between two groups were compared. Results The SBP, DBP, septal, left ventricular end - diastolic diameter, CRP, CK - MB and BNP of A group were higher than those of B group （ P 〈 0.05 ）. Conclusion The SBP, DBP and left ventricular end -diastolic diameter of EH combined with heart failure patients were higher than those of EH patients, septal was thicker. To control blood pressure and heart failure is important to improve the prognosis of patients with EH combined with heart failure.
