摘  要:目的探讨鼠神经生长因子(mNGF)联合高压氧对缺氧缺血性脑病(HIE)新生儿血清神经元特异性烯醇化酶(NSE)和髓鞘碱性蛋白(MBP)的影响及临床疗效。方法选取2010—2012年我院收治的足月HIE患儿92例,将其随机分为联合治疗组和常规治疗组,各46例;另选取同期我院出生的正常足月新生儿30例作为对照组。两组均采取综合治疗措施,联合治疗组在综合治疗措施基础上采取肌肉注射mNGF和高压氧治疗。于治疗前后测定所有受试者血清NSE、MBP水平,并评价各组出生后3、10、14 d新生儿行为神经测定(NBNA)评分。结果治疗前联合治疗组和常规治疗组NSE和MBP水平均高于对照组(P<0.05);联合治疗组和常规治疗组NSE和MBP水平比较,差异无统计学意义(P>0.05)。治疗后联合治疗组NSE和MBP水平均低于常规治疗组(P<0.05)。出生后3 d联合治疗组和常规治疗组NBNA评分比较,差异无统计学意义(P>0.05);出生后10、14 d联合治疗组NBNA评分均高于常规治疗组(P<0.05)。结论应用mNGF联合高压氧治疗HIE新生儿能显著改善患儿神经系统功能障碍,减少后遗症,值得临床应用。Objective To observe the effect of mouse nerve growth factor (mNGF) on serum neuron specific enolase (NSE) and myelin basic protein (MBP) in neonates with hypoxie ischemic encephalopathy (HIE). Methods 92 neonates with HIE admitted to our hospital from 2010 to 2012 were randomly divided into combination treatment group and routine treatment group, each of 46 cases; 30 healthy neonates at she same time were selected as control group. All neonates were given compre- hensive treatment measures, combination treatment group was given mNGF and hyperbaric oxygen alone. The level of serum NSE and MBP in all subjects before and after treatment were detected. Neonatal behavioral neurological assessment (NBNA) score a- mong 3 groups were evaluated 3, 10, 14 days after birth. Results Before treatment, the level of NSE and MBP in combination treatment group and routine treatment group were higher than those of control group (P 〈 0. 05 ) ; the level of NSE and MBP be- tween combination treatment group and routine treatment group showed no significant differences ( P 〉 O. 05 ) . After treatment, the level of NSE and MBP in combination treatment group were lower than those of routine treatment group (P 〈0.05) . After 3 days of birth, the NASA scores between combination treatment group and routine treatment group showed no significant differences (P 〉0.05) ; after 10, 14 days of birth, the NASA score in combination treatment group was higher than that of routine treat- ment group ( P 〈 0.05 ). Conclusion mNGF and hyperbaric oxygen in treating neonates with hypoxic ischemic encephalopathy can significantly improve nervous system dysfunction and reduce sequelae, being worthy of promotion.
