天花粉宫颈注射联合米非司酮终止瘢痕子宫中晚期妊娠的临床应用

摘　　要：目的探讨天花粉宫颈注射联合米非司酮终止瘢痕子宫中晚期妊娠的临床疗效。方法选择2011年1月—2012年12月在荆门市第一人民医院产科就诊的瘢痕子宫中晚期妊娠引产的孕妇20例为研究组,宫颈注射天花粉蛋白注射液2.4 mg,同时口服米非司酮片25 mg,1次/8 h,共3次;选择同时期正常中晚妊娠期无合并症要求引产的孕妇20例为对照组,羊膜腔穿刺注入乳酸依沙吖啶注射液(利凡诺注射液)100 mg,同时口服米非司酮片25 mg,1次/8 h,共3次。比较两组孕妇胎儿娩出时间、产后出血量及组织残留清宫、不良反应发生率。结果对照组与研究组胎儿娩出时间〔(54±20)h与(55±16)h〕及产后出血量〔(110±31)ml与(108±27)ml〕比较,差异均无统计学意义(P>0.05)。对照组组织残留清宫17例,研究组无一例组织残留清宫。对照组发热1例,无皮肤红疹瘙痒、咽痛、关节疼痛;研究组发热16例,皮肤红疹瘙痒10例,咽痛3例,关节疼痛1例;研究组发热率较对照组升高(χ2=23.02,P<0.05)。结论天花粉宫颈注射联合米非司酮终止瘢痕子宫中晚期妊娠,是安全、有效、创伤小的引产方法,值得在临床上推广应用。Objective To explore the clinical curative effect of cervical injection of trichosanthin combined with mife- pristone in termination of late pregnancy of scarred uterus. Methods A total of 20 late pregnant women with scarred uterus who had induced labor from January 2011 to December 2012 （ study group） were treated with Triehosanthin injection （2.4 rag） com- bined with oral mifepristone （25 mg/8 h, 3 times）. Twenty healthy late pregnant women （control group） who had induced la- bor were given rivanol injection （100 mg） by amniocentesis and oral mifepristone （25 mg/8 h, times）. Time of disengage- ment, postpartum hemorrhage, tissue residue curettage, adverse reactions were compared between 2 groups. Results There was not significant difference in time of disengagement [ （ 54 ± 20 ） h vs. （ 55 ± 16 ） h ] and postpartum hemorrhage [ （ 110 ±31 ） ml vs. （ 108 ±27） ml] between 2 groups （P 〉0. 05）. Seventeen of control group had tissue residue curettage, none in study group had. One had fever, none had skin rash itching, sore throat, joint pain in control group, and 16, 10, 3, 1, re- spectively, in study group. The rate of fever of study group was higher than that of control group （χ2 =23.02, P 〈0. 05）. Conclusion Triehosanthin injection combined with mifepristone in termination of late pregnancy of scarred uterus, safe, effec- tive and with smaller injuries, is worthy of popularization in clinic.
