摘 要:目的检测特应性皮炎(AD)患儿外周血单个核细胞(PBMCs)中巨噬细胞游走抑制因子(MIF)、IL-17和IL-23 mRNA表达水平及血清水平,并探讨其相关性。方法选择2010年4—11月在我院门诊就诊的36例AD患儿为病例组,同期在我院体检正常儿33例为对照组,采用实时荧光定量反转录聚合酶链反应(RT-PCR)检测两组受试者PBMCs中MIF、IL-17和IL-23 mRNA表达水平,酶联免疫吸附试验检测其血清MIF、IL-17和IL-23水平。结果病例组PBMCs中MIF、IL-17和IL-23 mRNA表达水平分别为(5.93±2.35)、(6.76±1.87)和(3.21±0.73),对照组分别为(1.83±0.69)、(1.77±0.51)和(0.92±0.22),病例组PBMCs中MIF、IL-17和IL-23mRNA表达水平均高于对照组(P<0.01)。病例组血清MIF、IL-17和IL-23水平分别为(34.92±8.73)μg/L、(33.29±5.70)ng/L和(54.62±9.69)ng/L,对照组分别为(9.80±2.21)μg/L、(11.71±2.08)ng/L和(18.97±4.39)ng/L,病例组血清MIF、IL-17和IL-23水平均高于对照组(P<0.01)。线性相关分析结果显示,AD患儿PBMCs中MIF mRNA表达水平(r=0.418,P=0.011)及血清MIF水平(r=0.419,P=0.011)与疾病严重程度(SCORAD)评分均呈正相关;PBMCs中MIF mRNA表达水平与IL-17 mRNA、IL-23 mRNA表达水平均呈正相关(rIL-17 mRNA=0.395,P=0.017;rIL-23 mRNA=0.422,P=0.010);血清MIF水平与IL-17、IL-23水平均呈正相关(rIL-17=0.407,P=0.014;rIL-23=0.375,P=0.024)。结论 MIF在AD患儿中高表达,且与IL-17、IL-23密切相关,炎性因子间的相互作用可能共同参与了AD的发生、发展。Objective To detect the expressions of macrophage migration inhibitory factor(MIF),IL-17,IL-23 in peripheral blood mononuclear cells(PBMCs) and serum levels,and their correlation in children with atopic dermatitis(AD).Methods Thirty-six AD children who visited this hospital from April to November 2010 were enrolled as study group,33 healthy children as control group.The expressions of MIF,IL-17,IL-23 in PBMCs were detected by real-time quantitative RT-PCR,levels of serum MIF,IL-17,IL-23 by enzyme-linked immunosorbent assay.Results The expressions of MIF,IL-17,IL-23 in PBMCs were higher in study group[(5.93±2.35),(6.76±1.87),(3.21±0.73),respectively]than in control group[(1.83±0.69),(1.77±0.51),(0.92±0.22),respectively];the serum levels of MIF,IL-17,IL-23 higher in study group[(34.92±8.73)μg/L,(33.29±5.70)ng/L,(54.62±9.69)ng/L,respectively]than in control group[(9.80±2.21)μg/L,(11.71±2.08)ng/L,(18.97±4.39)ng/L,respectively].By linear correlation analysis,MIF mRNA expression in PBMCs,serum MIF level were positively correlated with disease severity(SCORAD) score(r=0.418,P=0.011;r=0.419,P=0.011);MIF mRNA expression in PBMCs positively correlated with expressions of IL-17,IL-23(rIL-17 mRNA=0.395,P=0.017;rIL-23 mRNA=0.422,P=0.010);serum MIF level positively correlated with IL-17,IL-23(rIL-17=0.407,P=0.014;rIL-23=0.375,P=0.024).Conclusion In AD children,MIF expression is high and closely correlated with IL-17,IL-23.The interaction between inflammatory cytokines may be involved in the occurrence and development of AD.
