摘  要:目的评价万古霉素对金黄色葡萄球菌的最低抑菌浓度(MIC)值是否存在漂移,并为临床抗感染治疗提供参考依据。方法收集医院2006-2011年从临床标本中分离的286株非重复金黄色葡萄球菌,琼脂稀释法测定万古霉素对菌株的MIC,计算MIC50、MIC90、MIC几何均值,以苯唑西林对金黄色葡萄球菌MIC值确认耐甲氧西林金黄色葡萄球菌(MRSA),比较MRSA与对甲氧西林敏感金黄色葡萄球菌(MSSA)万古霉素MIC值的差异,应用SPSS 19.0进行统计分析。结果收集到的286株金黄色葡萄球菌均对万古霉素敏感;6年金黄色葡萄球菌对万古霉素MIC几何均值依次为1.116、1.118、1.042、1.109、1.109、1.086,未发现万古霉素MIC值上升趋势;2006-2011年检出MRSA株数依次为11、20、23、27、16、35株,检出率分别为40.7%、43.5%、46.0%、60.0%、29.6%、55.4%,差异有统计学意义(P<0.05);MRSA与MSSA对万古霉素MIC几何均值差异有统计学意义(P<0.05)。结论医院2006-2011年临床分离株万古霉素MIC几何均值无逐年上升趋势,未发现万古霉素对金黄色葡萄球菌的MIC值漂移。OBJECTIVE To evaluate the change of minimum inhibitory concentration (MIC) value of vancomycin against Staphylococcus aureus so as to provide guidance for the clinical treatment of infections .METHODS From 2006 through 2011 ,totally 286 strains of non-fermenting S .aureus were isolated from the clinical specimens ,then the MIC of the strains against vancomycin was determined by using agar dilution method ,the geometric means of MIC50 ,MIC90 ,and MIC were calculated ,the methicillin-resistant S .aureus (MRSA) was confirmed with the use of MIC value of oxacillin against S .aureus ,the difference in the MIC value of vancomycin against MRSA and the methicillin-susceptible S .aureus (MSSA) was observed and compared ,and the statistical analysis was performed by using SPSS 19 .0 software .RESULTS All the 286 strains of S .aureus were susceptible to vancomycin .The geometric mean MICs of vancomycin against S .aureus in the six years were 1 .116 ,1 .118 ,1 .042 ,1 .109 ,1 .109 , and 1 .086 ,respectively .The number of the strains isolated during 2006-2011 were 11 ,20 ,23 ,27 ,16 ,and 35 , respectively ,and the detection rates were 40 .7% ,43 .5% ,46 .0% ,60 .0% ,29 .6% ,55 .4% ,respectively ,the differences were significant(P&lt;0 .05) .The difference in the geometric mean MIC value of vancomycin between MRSA and MSSA was significant(P&lt;0 .05) .CONCLUSION From 2006 to 2011 ,the geometric mean MIC value of vancomycin against the clinical isolates does not show an upward trend ,and the drift in the MIC value of vanco-mycin against S .aureus has not been discovered .
