摘　　要：目的分析综合医院住院患者抗菌药物日使用的变化趋势,评价抗菌药物管理临床应用综合整治效果。方法采用横断面调查方法,调查2008-2012年每年某一天在院患者抗菌药物的使用情况,调查内容包括患者一般情况、是否使用抗菌药物、用药目的、具体药物名称及用法用量、病原学培养等。结果 2008-2012年分别调查701、700、759、737和861例,使用抗菌药物的例数分别为333、350、303、257和278例,使用率分别为47.50%、50.92%、39.92%、34.87%和32.29%,呈下降趋势;外科系统使用率由2008年65.75%下降至2012年36.02%,治疗性用药比例由41.15%上升至52.98%,预防性用药由52.08%下降至37.32%,用单药比例67.67%上升至83.58%,治疗性用药病原学送检率由20.65%上升至33.33%。结论该综合性医院抗菌药物临床应用综合整治取得明显效果,预防性用药更趋合理。OBJECTIVE To analyze the variation trend of daily antimicrobial use among inpatients in a general hospi-tal and evaluate the effect of the comprehensive improvement of clinical application of antimicrobials .METHODS Daily antimicrobial use investigation was carried out by cross-sectional investigation method among inpatients each year from 2008 to 2012 . The research contents include general information of patients , whether they used antimicrobials ,drug use purposes ,specific drug name and usage and dosage ,etiology culture ,etc .RESULTS The number of cases investigated from 2008 to 2012 reached 701 ,700 ,737 ,759 and 861 each year ,of which the cases using antimicrobials were 333 ,350 ,303 ,257 and 278 respectively .The yearly rate of daily antimicrobial use was 47 .50% ,50 .92% ,39 .92% ,34 .87% and 32 .29% ,showing a declining curve .The figure from surgical wards came down from 65 .75% in 2008 to 36 .02% in 2012 while the proportion of therapeutic drugs use rose from 41 .15% to 52 .98% .The percentage of prophylaxis drugs use was 52 .08% in 2008 ,dropping to 37 .32% in 2012 . Meanwhile ,the proportion of single drug usage rose from 67 .67 % to 83 .58 % and the etiological examination rate of therapeutic drugs rose from 20 .65% to 33 .33% .CONCLUSION The comprehensive improvement of clini-cal application of antimicrobials in this general hospital is apparent .The prophylaxis drugs use tends to be more rational .
