摘  要:目的研究不同类型的儿童肺部感染外周血中超敏C-反应蛋白(hsCRP)、降钙素原(PCT)以及T淋巴细胞亚群水平,通过与正常儿童的比较,以了解其与患儿发生肺炎的关系,为临床资料提供参考依据。方法随机选择2010年8月-2013年8月诊断为肺炎的患儿103例,按照不同的类型分为病毒感染性肺炎41例、细菌感染性肺炎33例及支原体属感染性肺炎29例,同时选择同期查体的健康儿童48名作为对照组,比较4组患者各观察指标的差异,数据采用SPSS17.0统计软件进行分析。结果细菌感染患儿的PCT水平均高于对照组(P<0.05),支原体属和病毒感染性肺炎患儿与对照组差异无统计学意义;细菌感染和支原体属感染性肺炎患儿的hsCRP高于病毒感染组和对照组,差异有统计学意义(P<0.05);3组肺炎感染组患儿的CD3+、CD4+、CD4+/CD8+、CD4+CD25+调节性T细胞水平较对照组差异均有统计学意义(P<0.05),各感染组之间比较差异无统计学意义;细菌感染性肺炎和支原体感染性肺炎CD8+淋巴细胞水平高于其余两组,差异有统计学意义(P<0.05)。结论 3个指标对于儿童肺部感染诊断都有一定的参考价值,尤其是PCT可以作为早期诊断细菌性感染的灵敏指标。OBJECTIVE To investigate the levels of high-sensitivity C-reactive protein (hs-CRP ) , procalcitonin (PCT ) and T lymphocyte subsets in children pneumonia with various pathogens and compare the differences between them and healthy children for understanding the relation between these and the incidence of children pneu-monia ,so as to provide reference for clinic .METHODS A total of 103 cases of children with pneumonia from Aug . 2010 to Aug .2013 were divided into viral pneumonia group (41 cases) ,bacterial pneumonia group (33 cases) and mycoplasma pneumonia group (29 cases) and 48 health children were chosen as control group .Various indicators were compared among various groups .SPSS 17 .0 software was used for statistical analysis .RESULTS The levels of PCT in pneumonia group were significantly higher than that in control group (P&lt;0 .05) .The levels of PCT in viral pneumonia group and mycoplasma pneumonia group were not significantly different from that in control group .The levels of hs-CRP in bacterial pneumonia group and mycoplasma pneumonia group were significantly higher than those in the viral pneumonia group and control group (P&lt;0 .05) .The levels of CD3 + ,CD4 + ,the rati-o of CD4 + /CD8 +and the level of CD4 +CD25 + Treg in bacterial pneumonia group ,mycoplasma pneumonia and viral pneumonia group were significantly different from those in the control group (P&lt;0 .05) .However ,there were no significant differences among those three pneumonia groups .The levels of CD8 + lymphocytes in bacterial pneumo-nia group and mycoplasma pneumonia group were higher than those in the viral pneumonia group and nomal group (P&lt;0 .05) .CONCLUSION Hs-CRP ,PCT and T lymphocyte subsets are valuable to a certain extent to identify early childhood pneumonia caused by different pathogens .PCT can be used as a sensitive indicator for the early diagnosis of bacterial infection in children .
