氧化蛋白产物在临床脓毒症中的应用

摘　　要：目的探讨脓毒症患者血清氧化蛋白产物水平(AOPPs)和炎症因子水平(TNF-α、IL-Ⅰ、IL-6)与脓毒症预后的关系。方法选取2011年1月-2012年1月入住医院ICU的70例脓毒症患者按病情严重程度分为脓毒症组23例、严重脓毒症组38例和脓毒症休克组9例与12名健康对照者一起作为试验对象;分别采用光谱分析和ELISA法对受试对象血清进行AOPPs和炎症因子测定,并采用急性生理学与慢性健康评分Ⅱ(APACHEⅡ)对患者病情程度进行评分。结果随着病情程度的加重,AOPPs水平升高明显,健康对照组数值为31.24μmol/L,脓毒症休克时高达67.83μmol/L,超过了健康对照组的两倍,各组之间测量值差异均有统计学意义(P<0.05);脓毒症组TNF-α和IL-6水平最高,分别有40.19ng/L和27.03ng/L,脓毒症休克组IL-Ⅰ测量值高达29.33ng/L。结论脓毒症时AOPPs水平升高,体内存在严重的氧化应激状态;脓毒症患者AOPPs水平可反应患者的病情严重程度。OBJECTIVE To discuss the relationship between the levels of advanced oxidation protein products （AOPPs） and inflammatory cytokines （TNF-α,IL-Ⅰ and IL-6） and the prognosis of patients with sepsis . METHODS A total of 70 sepsis patients who were hospitalized the ICU from Jan 2011 to Jan 2012 were divided into the sepsis group with 23 cases ,the severe sepsis group with 38 cases ,and the sepsis shock group with 9 cases according to the severity of disease ,meanwhile 12 healthy people were set as the study controls ,then the spectral analysis and ELISA were respectively performed to determine the levels of AOPPs and inflammatory cytokines of the participants ,and the scoring of illness severity was conducted with the use of acute physiology and chronic health evaluation Ⅱ（APACHEⅡ ） .RESULTS With the severity of illness aggravating ,the level of AOPPs was increased significantly ,which was 67 .83 μmol/in the patients with septic shock ,two times higher than 31 .24μmol/L in the healthy controls ,the difference in the measurement value between the two groups was significant （P＆lt;0 .05） .The levels of TNF-αand IL-6 of the sepsis group were 40 .19 ng/L and 27 .03 ng/L ,respectively ,which were the highest ;the level of IL-1 of the septic shock group reached up to 29 .33 ng/L .CONCLUSION The level of AOPPs is elevated in the patients with sepsis ,indicating the severe oxidative stress condition of human body ;the level of AOPPs can reflect the severity of sepsis .
