实质层单疱角膜炎的治疗体会

摘　　要：实质层单疱角膜炎由单疱病毒感染所致,本病病程漫长,病情缠绵,反复发作。临床上多表现为慢性炎症过程,病变多在角膜中央部基质层,易并发虹睫炎和角膜穿孔,严重影响视力,是重要致盲眼病之一。其发病与机体免疫力低下有密切关系。目前虽有中西医多种治疗方法,但都未能较有效地防止其复发。笔者总结多年的实践经验,提出本病的主要病因病机为肝经郁热挟湿邪上犯清窍,立清肝化湿,退翳明目为主要治则。特别强调在治疗过程中必须保护脾之阳气,用药宜淡渗利化,忌用苦寒攻下,否则非但湿邪不去,反致损伤脾之阳气,使病情更复杂难愈.晚期则强调健脾益气,固肾明目,提高机体的免疫功能,既可促进溃疡的愈合和组织水肿的吸收,又能巩固疗效,防止复发。同时对连续追踪观察一年以上的13例作了分析。Simple herpetic keratitis (profundal type) is a chronic inflammatory eye disease with repeated occurrence which can lead to blindness. The author considers that this disease is due to heat and dampness retention in liver affecting upper orifices. The major principle of the treatment should be not only clearing away liver-heat and eliminating dampness, removing nebula to improve visual acuity but also protecting spleen-yang. Drugs selected should be those of bland nature, capable of eliminating dampness, and those of bitter taste and cold nature should not be used. In the late stage, attention should be paid to benifiting qi and invigorating the spleen, strengthening the kidney to improve visual acuity, which helps to enhance the immunologic function, promote the healing of ulcer and prevent from recurrence.
