摘  要:糖尿病心肌病(DCM)是一种特异性心肌病,主要是因高血糖导致的心脏微血管病变和心肌代谢紊乱,特点为左心室肥厚和舒张功能障碍,伴或不伴收缩功能不全。目前认为DCM的发生发展涉及多种机制,如能量代谢紊乱,电生理改变,氧化应激,RAS异常激活等。DCM患者易合并多种心律失常,且心电图常显示T波异常、QT间期延长等现象,这与电生理机制异常密切相关。DCM的电生理改变主要涉及Ca2+超载与ATP敏感性钾通道功能异常。Diabetic eardiomyopathy ( DCM ), characterized by left ventricular hypertrophy and diastolic dysfunction,with or without systolic dysfunction, is a specific cardiomyopathy. The cardiac microangiopathy and metabolic disorders in cardiac muscles is mainly caused by hyperglycemia. The pathogencsis mechamisms is considered to involve energy metabolic disorders, alterations of electmphysiology, oxidative stress, abnor- malties of activated RAS, and so on. At present. DCM patients get arrhythmogenic easily, and ECG usually shows T wave abnormality, QT interval prolongation, which are closely related to abnormal mechanisms of electrophysiology. The electmphysiology alterations are mainly caused by calcium overload and dysfunction of KATP.
