消脓汤用于术后腹腔残余感染的临床观察

摘　　要：目的探讨消脓汤联合抗生素治疗术后腹腔残余感染的临床效果。方法选取兰州大学第一医院2011年1月至2012年12月收治的术后腹腔残余感染患者60例作为研究对象,按随机数字表法随机分为联合消脓汤组(n=30)和单纯抗生素组(n=30)。联合消脓汤组给予抗生素(头孢哌酮钠他唑巴坦钠)联合消脓汤治疗,单纯抗生素组单纯应用抗生素治疗。观察并比较治疗后两组患者血象复常时间、体温恢复正常时间,腹痛消失时间、脓肿消失时间,患者住院时间及疗效。结果与单纯抗生素组相比,联合消脓汤组患者血象复常时间、体温复常时间、腹痛消失时间、脓肿消失时间及住院时间均显著缩短(t=16.331,4.739,10.756,6.224,5.464,P<0.05);联合消脓汤组临床疗效显著优于单纯抗生素组(z=3.554,P<0.05)。结论抗生素联合消脓汤治疗术后腹腔残余感染较单纯应用抗生素治疗更有效。Objective To investigate the effects of the pus-dissolving decoction for postoperative intra- abdominal residual infection. Methods A total of 60 patients with confirmed postoperative intra-abdominal re- sidual infection in the First Hospital of Lanzhou University were randomly divided into pus-dissolving decoction combination group （n = 30） and single antibiotics group（ n = 30 ）, with 30 cases in each group. Both groups were given antibiotics （ cefoperazonc sodium and tazobactam sodium）, while the pus-dissolving decoction combination group was also given the pus-dissolving decoction. Hemogram recovery time, body temperature return to normal time, abdominal ache disappearance time, abscess disappearance time, length of hospital stay and e^cacy were observed. Results Compared with the single antibiotics group, hemogram recovery time, body temperature returned to normal time, abdominal ache disappearance time, abscess disappearance time and length of hospital stay of the pus-dissolving decoction combination group were significantly shorter（ t = 16. 331,4. 739,10. 756, 6. 224,5. 464 ,P 〈 0.05 ）;the curative effect of pus-dissovling decoction combination group was significantly better than the single antibiotics group （z = 3. 554, P 〈 0. 05 ）. Conclusion Antibiotics combined with pus- dissolving decoction has better curative effect on postoperative intra-abdominal residual infection than antibiotic therapy alone.
