摘  要:目的观察噻唑烷二酮类药物罗格列酮对糖调节受损患者红细胞流变行为的影响。方法选取新诊断的糖调节受损患者120例,随机分为A组60例和B组60例,选取同时期健康体检者40例作为对照组。A组给予生活方式干预,B组给予生活方式干预联合罗格列酮4 mg/d口服,疗程12个月。治疗前及治疗后第3,6,12个月测体质量、空腹血糖(FBG)、餐后2 h血糖(2hPG)、空腹胰岛素(FINS)、糖化血红蛋白(HbA1c)、红细胞刚性指数、红细胞聚集指数,计算胰岛素抵抗指数(HOMA-IR)。结果 A组及B组各指标显著高于对照组(P均<0.05)。治疗后2组FBG、2hPG、HbA1c、红细胞刚性指数、红细胞聚集指数及B组体质量、FINS和HOMA-IR显著降低;6个月及12个月时,B组体质量、FBG、2hPG、HbA1c、FINS、HOMA-IR、红细胞刚性指数、红细胞聚集指数显著低于A组(P均<0.05),12个月时B组红细胞刚性指数、红细胞聚集指数较同组6个月时显著降低(P均<0.05)。结论糖调节受损患者坚持生活方式干预联合噻唑烷二酮类药物治疗(至少6个月),在降低血糖的同时,可通过多种途径改善红细胞流变行为,可预防血管并发症的发生、发展。Objective It is to observe the effects of thiazolidinedione drug rosiglitazone on the theological behavior of e- rythrocytc in patients with impaired glucose regulation. Methods 120 patients with impaired glucose regulation newly diag- nosed were randomly divided into group A ( n = 60) and group B ( n = 60) , while another 40 healthy individuals were selected as the control group. Lifestyle interventions targeted the patients in group A for 12 months, while lifestyle interventions com- bined with rosiglitazone(4mg/d) in group B. Body weight, fasting plasma glucose (FPG) , 2 -hour postprandial blood glu- cose (2hPG) , fasting insulin(FINS) , hemoglobin A IC (HbA1C) , erythrocyte rigidity index and erythrocyte aggregation index were measured while HOMA -IR were calculated before and 3, 6, 12 months after treatment. Results Before the treatment, all indicators in group A and B were significantly higher than in the control group ( P 〈 O. 05 ). After the treatment, FPG, 2hPG, HbA1 C, erythrocyte rigidity index and erythrocyte aggregation index in group A and B and body weight, FINS, HOMA - IR in group B were significantly reduced(P 〈0.05). The body weight, FPG, 2hPG, HbA1C, FINS, HOMA -IR, erythro- cyte rigidity index and erythrocyte aggregation index in group B were significantly lower than group A at the 6th and 12th month after treatment( P 〈 0.05 ) , while erythrocyte rigidity index and erythrocyte aggregation index in group B at the 12th month were significantly lower than that at the 6th after treatment( P 〈 0.05 ). Conclusion Continuous lifestyle interventions combined with the thiazolidinedione drug administration( more than 6 months) not only can reduced blood glucose, but also improve the rheo- logical behavior of erythrocyte in patients with impaired glucose regulation by multiple pathways in order to prevent the occur- rence and development of the blood vessel complications.
